经尿道等离子前列腺电切术与前列腺剜除术的临床疗效比较

Comparison on the clinical efficacy of transurethral plasmakinetic resection of prostate and enucleation of prostate

目的：分析对前列腺增生患者给予经尿道等离子前列腺电切术、前列腺剜除术的治疗效果及安全性。方法选择2011年2月~2013年11月前列腺增生患者202例为研究对象，随机分为两组。Ⅰ组患者给予经尿道等离子前列腺电切术治疗，Ⅱ组患者给予经尿道等离子前列腺剜除术，比较两组前列腺手术患者手术时间、出血量和并发症发生率等。结果剜除术组患者手术中出血量少，并发症发生率少，与电切术组患者比较，差异有统计学意义（P＜0.05）；剜除术组与电切术组手术时间、RUV及IPSS评分比较，差异无统计学意义（P＞0.05）。结论对前列腺增生患者手术治疗使用经尿道等离子前列腺剜除术的效果更好，不但能够保证治疗效果，还可以减少创伤和并发症，值得应用。

Objective To analyze the therapeutic effect and safety of transurethral plasmakinetic resection of prostate and enucleation of prostate for patients with prostatic hyperplasia. Methods 202 patients with prostatic hyperplasia from Feb 2011 to Nov 2013 were selected as research objects and randomly assigned to two groups. GroupⅠ was given transurethral plasmakinetic resection of prostate while Group Ⅱ was given transurethral plasmakinetic enucleation of prostate. The operation duration, blood loss and complication rate were observed and compared between the two groups. Results The enucleation group had less intraoperative blood loss and lower complication rate than the resection group,with statistical significance（P 〈 0.05）. The enucleation group and the resection group had no significant difference in operation duration, RUV and IPSS score,without statistical significance（P 〉0.05）. Conclusion Transurethral plasmakinetic enucleation of prostate demonstrates good effects on patients with prostatic hyperplasia. It does not only guarantee therapeutic effects but also reduce trauma and complications, which is worthy of clinical application.
