循证护理在毕Ⅰ式胃大部切除术后的应用价值

Application of evidence-based nursing combined with family education in treatment of patients with billroth typeⅠ gastrectomy

目的探讨循证护理在毕Ⅰ式胃大部切除术后的应用价值。方法选取2010年5月—2013年5月行毕Ⅰ式胃大部切除术的患者88例,随机分为观察组与对照组各44例。对照组给予常规护理,观察组给予循证护理。观察2组护理前与护理4周后的焦虑情况、术后并发症及护理满意度。结果护理前2组焦虑情况无显著差异(P>0.05),护理4周后观察组正常及轻度焦虑例数显著多于对照组(P<0.05),中、重度焦虑例数显著少于对照组(P<0.01);观察组并发症总发生率为6.82%,对照组总发生率为61.36%,2组比较差异显著(P<0.01);2组的护理总满意度分别为97.73%和61.36%,观察组显著优于对照组(P<0.01)。结论循证护理可显著改善毕Ⅰ式胃大部切除术后患者的焦虑状况,减少术后并发症的发生,提高护理满意度。

Objective To expolre the value of evidence-based nursing in postoperative treatment of patients with billroth tpye I subtotal gastrectomy.Methods 88 patients with billroth tpye I subtotal gastrectomy were randomly divided into observation group and control group.Obser-vation group was given evidence-based nursing combined with family education,while control group was given regular nursing.The anxiety,postoperative complications and satisfaction with nursing of 2 groups were observed before treatment and 4 weeks after intervention.Results There was no significant difference in anxiety between 2 groups before treatment （P 〉0.05）.After 4 weeks,incidence rates of normal and mild anxiety in observation were significantly higher than those of control group （P 〈0.05）while incidence rates of moderate and severe anxiety were signifi-cantly less than those of the control group （P 〈0.01）.Difference in overall incidence of complica-tions between 2 groups was significant,with 6.82% in observation group and 61.36% in control group,respectively（P 〈0.01）.Satisfactions with nursing between 2 groups was significantly dif-ferent,with 97.73% in observation group and 61.36% in control group,respectively.Conclusion Evidence-based nursing has the potential to release the anxiety of patients with Billroth tpye Ⅰsubtotal gastrectomy,reduce the postoperative complications and improve nursing qualification.
