Pontin在结肠癌中的表达及其意义

Expression of pontin and its clinical significance in colon cancer

目的通过检测Pontin在结肠癌组织、腺瘤组织和切缘黏膜组织中的表达,分析三者表达与结肠癌不同临床病理特征的关系,探讨其对预后的判断意义。方法收集86例结肠腺癌(腺癌组)、60例结肠腺瘤(腺瘤组)和60例结肠癌旁正常组织(切缘黏膜组),应用免疫组化SP法检测3组中Pontin蛋白的表达。结果 Pontin在腺癌组、腺瘤组、切缘黏膜组中阳性率分别为66.3%、31.7%和15%,Pontin表达的阳性率依次降低且差异极显著(P<0.01)。结肠腺癌组中Pontin的高表达与分化差、有淋巴结转移、Ⅲ+Ⅳ期、肿瘤直径>3 cm、浸润深度达浆膜及以外Ki-67高表达相关(P<0.01);与性别、年龄、炎细胞浸润、部位无关(P>0.05)。Pontin高表达患者的预后差(P<0.01)。结论 Pontin的高表达可能促进结肠癌的进展,检测Pontin蛋白的表达有助于判断结肠癌预后。

Objective To investigate the expression of Pontin in colon carcinoma,colon adenoma and normal colon mucosa,and to analyze the correlation of its expression with clinicopathologic features and the prognosis in colon carcinoma,in order to provide the theoretical support for progression and development of colon carcinoma.Methods The study included 86 cases of colon carcinoma,60 cases of colon adenoma and 60 cases of colon margin as marginal normal mucosa from Tangshan Gongren Hospital.In our study,we detected the expression of Pontin in the three groups by using the immunohistochemical methods.SAS 6.12 software was adopted to analyze the data.Comparison among groups was performed with x2 test,linear correlation and survival analysis were also used in the statistic analysis,there were significances in statistics when P ＜ 0.05.Results The positive rates of Pontin were 66.28％,31.67％ and 15.00％ in colon carcinoma,colon adenoma and marginal normal mucosa respectively.Positive rate fell in turn among the three groups,and there were significantly differences （x2 =41.5610,P ＜ 0.001）.The expression of Pontin was correlated with worse differentiation （x2 =6.2993,P ＜0.05）,lymph node metasis （x2 =4.2129,P ＜0.05,clinical stage Ⅲ ＋ Ⅳ （x2 =7.9082,P ＜ 0.01）,diameter ＞ 3 cm （x2 =19.7866,P ＜ 0.001）,invasive depth out of serosa （x2 =5.5702,P ＜ 0.05） and higher Ki-67 expression （x2 =13.9361,P ＜ 0.001）.But their expressions were not correlated with ages,sex,inflammation invasion and neoplasm location （P ＞ 0.05）.Higher expression of Pontin was associated with poor prognosis （P ＜ 0.01）.Conclusion The higher expression of Pontin promotes the development of colon carcinoma,and the detection of Pontin could be helpful for the prediction of prognosis in colon carcinoma.
