摘要
目的探讨彩色多普勒超声引导下活检结合造影在卵巢肿瘤诊断中的应用价值。方法选择2010年10月至2013年2月我院收治的经活检病理确诊的卵巢肿瘤患者180例(良性卵巢肿瘤120例,恶性卵巢肿瘤60例),所有患者都给予了二维超声、彩色多普勒超声与造影分析。结果良性肿瘤超声显示多为囊性,彩色超声显示囊壁少量点状血流;恶性肿瘤超声显示多为囊实性,彩色超声显示囊壁丰富血流。恶性肿瘤患者的动脉RI值明显高于良性肿瘤患者,对比差异有统计学意义(P<0.05)。恶性卵巢肿瘤患者的增强速率与半廓清时间明显高于良性卵巢肿瘤患者,而始增时间与达峰时间明显低于良性卵巢肿瘤患者,对比差异有统计学意义(P<0.05)。结论彩色多普勒超声引导下活检结合造影在卵巢肿瘤诊断中的应用可实时动态显示血流灌注情况,有助于提高超声对良恶性卵巢肿瘤的诊断及鉴别诊断能力。
Objective To investigate applying values of color Doppler ultrasound-guided biopsy combined with angiography in the diagnosis of ovarian tumors. Method Patients with ovarian tumor diagnosed by biopsy pathology(120 cases of benign ovarian tumor,60 cases of malignant ovarian tumor) were chosen in our hospital from October 2010 to February 2013,all of whom were given the analysis of two-dimensional ultrasound,color Doppler ultrasound and angiography. Results Ultrasound showed benign tumors are mostly cystic,color ultrasound showed cystic walls were with a few dot flow; ultrasound showed malignant tumor was mainly solid,color ultrasound showed cystic walls were rich with blood flow. Arterial RI values of patients with malignant tumor were obviously higher than those of patients with benign tumor,compared difference was statistically significant(P〈 0. 05). For the patients with malignant ovarian tumor,the enhancement rate and half clearance time was significantly higher than those of patients with benign ovarian tumor,and the enhancement beginning time and the peak time was significantly lower than that of patients with benign ovarian tumors,with statistically significant difference(P〈 0. 05). Conclusion The application of color Doppler ultrasound-guided biopsy combined with angiography in the diagnosis of ovarian tumors can offer real-time dynamic display of perfusion,which helps to improve the ability of diagnosis and identification for the benign and malignant ovarian tumors by ultrasonography.
