独活寄生汤与烫熨方联合透明质酸钠治疗膝关节骨性关节炎临床研究

Clinical Research of Duhuo Jisheng Decoction and Ironing Prescription Combined with Hyaluronic Acid Sodium for Knee Osteoarthritis

目的:观察独活寄生汤与"烫熨Ⅲ号"方联合透明质酸钠治疗膝关节骨性关节炎的疗效。方法:将92例膝关节骨性关节炎患者随机分为2组各46例。对照组予膝关节腔内注射透明质酸钠,观察组在关节腔内注射透明质酸钠的基础上,采用独活寄生汤配合"烫熨Ⅲ号"方治疗。比较2组的临床疗效,观察患者膝关节功能和生活质量的变化。结果:临床疗效总有效率观察组为95.7%,对照组为71.7%,2组比较,差异有显著性意义(P<0.01)。治疗前,2组Michel Lequesen法疼痛、最大步行距离和日常生活方面的分数及总分分别比较,差异均无显著性意义(P>0.05);治疗后,2组各项分值与治疗前比较,差异均有非常显著性意义(P<0.01),2组间各项分值比较,差异均有非常显著性意义(P<0.01)。2组生活质量综合评定问卷(GQOLI-74)躯体功能、社会功能、心理功能、物质生活的分值与综合评分值比较,差异均有非常显著性意义(P<0.001)。结论:独活寄生汤与"烫熨Ⅲ号"方联合透明质酸钠治疗膝关节骨性关节炎,可改善患者的膝关节功能和生活质量,提高临床疗效,且不良反应少,综合效价高。

Objective： To study the clinical efficacy of Duhuo Jisheng decoction and ironing prescription combined with hyaluronic acid sodium for knee osteoarthritis. Methods： Ninety-two cases were equally divided into the observation group and the control group. The observation group with 46 patients received Duhuo Jisheng decoction and ironing prescription combined with hyaluronic acid sodium. The control group with 46 cases was treated with hyaluronic acid sodium alone. The curative effect of two groups was compared. The changes of the knee joint function and quality of life were observed. Results= The total effective was 95.7% in the observation group, and was 71.7% in the control group, the difference being significant（P〈 0.01）. Before treatment, the differences of the scores of pain, the biggest walking distance, daily activities and the total scores between the two groups were insignificant （P 〉 0.05）. After treatment, the observation indexes were all improved（P 〈0.001compared with those before treatment）, the difference of the indexes between the two groups was significant after treatment（P〈0.05）. The differences of somatic function, mental function, social function and material life condition were significant between the two groups after treatment（P〈 0.001）. Conclusion.. Duhuo Jisheng decoction and ironing prescription Ⅲ combined with hyaluronic acid sodium can obviously improve the symptoms and quality of life of knee osteoarthritis, which have synergistic action on curative effectand have less side effects.
