摘要
目的:通过对比研究窒息新生儿的波幅整合脑电图(aEEG)与连续脑电图监测(cEEG),寻找其间的差异及相关性。方法:收集20i1年9月~2013年12月在我中心新生儿重症监护室住院的窒息新生儿142例,其中男109例,女33例。对人组患儿同步进行连续多导联的脑电图及aEEG监测,对每份脑电图记录分别进行cEEG和aEEG分析。结果:142份aEEG分析发现:正常113例,中度异常20例,重度异常9例。同一样本资料的cEEG:正常54例,轻度异常54例,中度异常25例,重度异常9例。其中aEEG正常组中:cEEG正常54例,轻度异常48例,中度异常11例;aEEG中度异常组中,cEEG轻度异常6例,中度异常14例;aEEG9例重度异常中cEEG亦为重度异常。142例患儿中cEEG发现26例伴异常尖波,8例电发作,aEEG仅发现7例电发作。结论:cEEG异常检出率高于aEEG。对伴有异常尖波的患儿,cEEG检出率明显高于aEEG组,对重度异常及电发作,cEEG与aEEG异常率一致性较高,对于重症患儿,aEEG仍具有较高的阳性检出率及应用价值。
Objective: To evaluate amplitude-integrated EEG (aEEG) in comparison with continu- ous EEG (cEEG) in neonates with asphyxia and to find the difference and correlation between them. Methods;Selected 142 patients with asphyxia in NICU department of our center from Sep 2011 to Dec 2013 were selected, including 109 male cases and 33 female cases. Continuous multi-channel EEG and aEEG were recorded and analyzed. Results: 113 of 142 aEEG were normal, 20 moderately abnormal and 9 markedly abnormal in aEEG. cEEG 54 were normal, 54 mildly abnormal, 25 moderately abnormal and 9 markedly abnormal in cEEG. 54 normal in cEEG, 48 mildly abnormal and 11 moderately abnormal of 113 normal in aEEGs;6 mildly abnormal in cEEG, 14 moderately abnormal of 20 mildly abnormal in aEEGs. Conclusion:cEEG abnormal rate detection is higher than aEEG's, egpecially for sharp wave detection in neonates with asphyxia. They have high consistency for severe abnormality and electrographic seizures. For serious neonatal,aEEG is very useful and has a high value of positive detection.
