疼痛的综合干预对宫颈癌患者术后疼痛及焦虑的影响

The effect of pain comprehensive intervention on postoperative pain and anxiety of patients with cervical cancer

目的探讨术前疼痛的综合干预对宫颈癌患者术后疼痛及焦虑的影响。方法将该院2012年9月～2013年9月90例宫颈癌手术患者随机分为对照组和实验组，两组各45例，两组患者都给予术前常规护理，实验组在此基础上给予疼痛的综合干预。采用焦虑量表及简明疼痛量表来比较两组患者术后焦虑情绪、疼痛程度及疼痛对日常生活活动的影响程度之间的差异。结果实验组患者术后焦虑情绪较术前明显减低（P〈0．05）：实验组术后4h疼痛程度、24h最强疼痛程度与对照组相比也明显降低（P〈0．05）；术后疼痛对实验组患者的体位转变、深呼吸／咳嗽及情绪方面的影响也明显小于对照组（P〈0．05）。实验组患者比对照组患者提前1天下床活动。结论疼痛的综合干预对宫颈癌患者术后焦虑情绪和疼痛有明显的缓解作用，这种术前疼痛干预方法可以为以后宫颈癌患者缓解疼痛提供有效的指导作用，值得在临床推广使用。

【Objective】To study the effects of preoperative nursing interventiononpostoperative pain and anxiety in patients with cervical cancer. 【Methods】In a randomized controlled study conducted between September 2010and September 2011, patients undergoing cervical cancer surgery in our hospital were randomly assigned to an experimental group（n =45） or control group（n =45）. The experimental group received routine care and preoperative nursing care for pain, while the control group received routine care only. Structured questionnaires including an anxiety scale, brief pain inventory were used to assess the patients＇ postoperative pain, anxiety and the influence of painon their daily life activities. 【Results】Patients in the experimental group experienced a significant decrease in postoperative anxiety（P〈 0.05）; They also had statistically significantly lower postoperative pain intensity for 4 hours after surgery and lower highest pain intensity within the first 24 hours after surgery compared with those patients who were in the control group（P〈 0.05）; The influence of pain on position changes, deep breathing/coughing, and moments of emotion in experimental group was statistically significantly lower than that of the control group（P〈 0.05）.The experimental group also started out of bed activities 1 day earlier.【Conclusions】Preoperative nursing intervention for pain had positive effects on patients undergoing cervical cancer surgery, the intervention used in this study could serve as a guide for nurses to improve the pain care of these cervical cancer patients.
