女性靶控输注瑞芬太尼丙泊酚i-gel喉罩置入研究

Research on i-gel LMA insertion with target controlled infusion remifentanil and propofol in women

摘要 目的对女性靶控输注瑞芬太尼和丙泊酚行i-gel喉罩置入进行研究。方法 150例拟行喉罩全麻女性患者,随机分为R1、R2、R33组,瑞芬太尼效应室靶浓度分别2.5、3.0、3.5 ng/ml,丙泊酚靶控输注3.5μg/ml。待患者意识消失且丙泊酚和瑞芬太尼的血浆和效应室浓度达平衡后置入i-gel喉罩。记录基础值(T0)、喉罩置入前即刻(T1)、喉罩置入后1 min(T2)、喉罩置入后3 min(T3)时的收缩压(SBP)、舒张压(DBP)、心率(HR),记录密封压,纤支镜声门暴露评分,喉罩置入时是否有体动、喉痉挛和呛咳发生情况,喉罩置入深度及是否术中知晓。结果 R1、R2、R33组喉罩首次置入成功率均为100%,有体动者分别为10例、2例、4例,无喉痉挛、呛咳、术中知晓。与T0时比较,3组T1、T2、T3时SBP、DBP、HR均降低;与T1比较,T2、T3时SBP、DBP、HR差异无统计学意义。139例(93%)患者门齿位于标记水平线以下。结论女性瑞芬太尼靶控输注2.5、3.0、3.5 ng/ml复合丙泊酚靶控输注3.5μg/ml可成功置入i-gel喉罩。93%的患者喉罩置入深度在标记水平线以下。i-gel喉罩置入对血流动力学无明显影响。 Objective To research i-gel laryngeal mask airway (LMA) insertion with target controlled infusion(TCI) remifentanil and propofol in women. Methods One hundred and fifty female patients scheduled for general anesthesia with LMA were randomly allocated to one of three groups (R1, R2, R3). Anesthesia was induced with TCI propofol (target plasma concentration 3.5μg/ml) and remifentanil. The target effect concentration of remifentanil was 2.5 ng/ml, 3.0 ng/ml and 3.5 ng/ml respectively in the three groups. SBP, DBP, HR at baseline(T0), immediately before insertion(T1), 1 min after insertion (T2) and 3 min after insertion (T3) were recorded. Laryngospasm and cough during insertion, awareness, insertion depth, oropharyngeal Leak pressure (OLP) and fibreoptic view were recorded. Results All patients finished insertion suc-cessfully after the first attempt. Body movement occurred in 10 patients, 2 patients and 4 patients respectively in group R 1, R2 and R3. No laryngospasm, cough and awareness were observed. SBP, DBP, HR at T1, T2 and T3 were lower than that at T0 in the three groups. SBP, DBP, HR at T2 and T3 were similar with that at T1. 139 (93%) patients were inserted with the teeth below the horizontal line. Conclusion i-gel LMA insertion is successful with TCI propofol 3.5 μg/ml and TCI remifentanil 2.5 ng/ml, 3.0 ng/ml and 3.5 ng/ml respectively in women, and 93% teeth below the horizontal line. i-gel in-sertion has no significant effect on hemodynamic parameters.
