腹腔镜技术治疗创伤性小肠破裂的临床体会

Evaluation on efficacy of laparoscopy in the treatment of traumatic rupture of small intestine

目的:总结腹腔镜技术治疗创伤性小肠破裂患者的临床疗效。方法:回顾分析68例创伤性小肠破裂患者的临床资料。根据术者经验,28例患者行腹腔镜手术(观察组),40例行开腹手术(对照组),对比两组手术时间、术后首次下床活动时间、术后首次肛门排气时间、疼痛情况及并发症发生率。结果:两组手术时间差异无统计学意义(P>0.05);两组术后首次下床活动时间、首次肛门排气时间、疼痛评分差异有统计学意义(P<0.01);术后并发症发生率两组差异有统计学意义(P<0.05)。结论:腹腔镜手术治疗创伤性小肠破裂有助于肠道功能的恢复,且并发症少,疼痛轻。

Objective：To summarize the efficacy of laparoscopy in the treatment of patients with traumatic rupture of small intestine. Methods：According to experience of operator,68 cases with traumatic rupture of small intestine were selected as the observation objects,in which 28 patients who underwent laparoscopic surgery were selected as the research group,and 40 cases with open surgery were selected as the control group. The operation time,the first ambulation time after operation,the first anal exhaust time,pain and incidence of complications of two groups were compared. Results：The operation time of the research group was（126. 89 ± 18. 06） min,and that of the control group was（125. 93 ± 19. 17） min（ P〈0. 05）. The first ambulation time after operation of the research group was（3. 28 ± 1. 07） d,and that of the control group was（5. 05 ± 1. 13） d（P〉0. 01）. The first anal exhaust time of the research group was（2. 07 ± 0. 92） d,and that of the control group was（3. 25 ± 1. 22） d（P〈0. 01）. The pain score of the research group was（4. 63± 1. 10）,and that of the control group was（6. 08 ± 1. 21）（ P〈0. 01）. The incidence of complications in the research group was7. 14%,and that in the control group was 27. 5%（P〈0. 05）. Conclusions：Laparoscopic surgery in the treatment of traumatic rupture of small intestine is helpful for the recovery of intestinal function with fewer complications and less pain.
