广东省不同地区麻风村休养员生存质量分析

Analysis on the quality of life of leprosy recuperating members in leprosy villages from different areas of Guangdong Province

目的分析比较广东省不同地区麻风村休养员生存质量差异。方法利用WHOQOL-BREF问卷对粤东、粤西、珠三角地区麻风村休养员生存质量进行分析,并以社区老人作为参照,统计方法采用2检验、方差分析。结果生存质量总评分在不同地区间存在总体差异(F=23.78,P<0.001),分值由高到低依次为珠三角(76.46±14.76)分、粤东(69.74±6.12)分和粤西(55.33±21.63)分,社区老人(78.77±12.36)分高于各地麻风村休养员。生存质量4个领域中,生理、社会、环境领域在不同地区间均存在总体差异(F值分别为12.85、13.67、10.46,均有P<0.05),尽管心理领域总体在各地区间无差异,但Q5(生活有乐趣)、Q6(生活有意义)及Q26(有消极感受)均存在总体差异。麻风畸残对生存质量存在影响(F=9.23,P=0.001),但Ⅰ级畸残和Ⅱ级畸残之间不存在统计学差异(P=0.257)。结论各地区麻风村休养员生存质量存在差异,但均低于社区人群,我们应进一步探讨造成差异的原因,进行有效干预。麻风畸残降低生存质量,应加强对麻风病人的畸残防护,不断提高麻风病人的生存质量。

Objective To analyze the quality of life （QOL） of leprosy recuperating members in leprosy villages from different areas of Guangdong Province. Methods WHOQOL-BREF was used to evaluate, the QOL of the leprosy recuperating members in the leprosy villages of eas, west, and Pearl River Delta of Guangdong Province. Old people in nearby community were served as the control group. Data were analysized by Chi-test and ANOVA. Results Three domains such as physiology, society and environment of WHOQOL BREF had the overall differences among all areas （ F value were 12. 85, 13.67 and 10. 46 respectively,P 〈0.05）,although mental domain had no overall difference, the item such as Q5 （good feeling of life）, Q6 （feeling of meaning of life） and Q26 （negative feelings such as blue mood, despair, anxiety, and depression） had overall differences among all areas. The total score of QOL was different among all areas （ F = 23.78, P 〈 0. 001 ）. The descending order was from Pearl River Delta（76.46 ± 14. 76）, Eastern Guangdong （69. 74 ±6. 12 ） to Western Guangdong （55. 33 ± 21.63）. Old people in community（78. 77 ± 12. 36） were higher than all of them. Leprosy deformity affected QOL of leprosy cases （ F = 9. 23, P = 0. 001 ）, but there was no statistical difference between level I and level 11 （P = 0. 257 ）. Conclusions QOL of leprosy recuperating members in leprosy villages from different areas of Guangdong is different and all are lower than that of the old people from community. Further approach is needed to discver the reasons for QOL differences and implement support effective interventions. Since leprosy deformity affects leprosy recuperating members＇ QOL, leprosy deformity prevention to increase their QOL.
