结直肠癌K-ras基因状态及P21 ras蛋白表达的意义 被引量:1

摘要 目的探讨结直肠癌K-ras基因状态和P21 ras蛋白表达及其与临床病理特征的关系。方法分别应用免疫组化Envision二步法检测160例结直肠癌组织芯片中P21 ras蛋白表达及PCR扩增、DNA直接测序法检测癌组织中K-ras基因12、13和61密码子的突变状态。结果 P21 ras蛋白阳性表达率为48.1%(77/160),淋巴结转移阳性组P21 ras蛋白阳性表达率(61.9%)显著高于无淋巴结转移组(39.2%),差异有统计学意义(P<0.05)。K-ras基因突变56例,突变率为35%,突变位点均位于12、13密码子,61密码子未发现突变。突变类型包括:GGT→GTT(17例)、GGT→GAT(12例)、GGT→AGT(15例)、GGT→TGT(8例)、GGC→GAC(4例)。淋巴结转移阳性组和Dukes'分期为C+D期的K-ras基因突变率分别为47.6%和39.5%,明显高于无淋巴结转移组(26.8%)和Dukes'分期为A+B期组(20.5%),差异有统计学意义(P<0.05)。其中K-ras基因突变型组P21ras蛋白阳性表达率(91.1%)明显高于K-ras基因野生型组(25.0%),差异有统计学意义(P<0.05)。Spearman等级相关分析显示,P21 ras蛋白表达与K-ras基因12密码子突变呈正相关(r=0.63,P<0.05)。结论 K-ras基因突变和P21 ras蛋白的过表达可能与结直肠癌淋巴结转移相关,P21 ras蛋白表达与K-ras基因12密码子突变具有一定相关性;K-ras基因突变与高的肿瘤分期有关。检测K-ras基因突变对评价结直肠癌患者临床病理特征、个体化治疗及预后有指导意义。
