氯胺酮为麻醉剂的无抽搐电休克治疗对难治性抑郁症患者血清BDNF水平的影响 被引量:10

摘要 目的 探讨以氯胺酮为麻醉剂的无抽搐电休克(MECT)治疗难治性抑郁症患者的疗效及与脑源性神经营养因子(BDNF)含量变化的相关性。方法 将60例难治性抑郁症患者随机分为氯胺酮组及丙泊酚组(各30例)分别使用氯胺酮、丙泊酚作为麻醉剂进行MECT治疗。所有患者在MECT治疗前及治疗1~8次后完成汉密尔顿抑郁量(HAMD)、认知功能及不良反应量表(TESS)评定,抽血测BDNF浓度。结果 两组在治疗后HAMD总分均明显下降(P〈0.01),而氯胺酮组在治疗第1次开始减分优于丙泊酚组(P〈0.01);氯胺酮组从治疗第4次开始血清BDNF浓度明显升高(P〈0.01),丙泊酚组治疗前后无明显变化;MECT治疗后血清BDNF浓度与HAMD总分呈显著负相关(r=-0.368,P〈0.01)。结论 氯胺酮作为MECT麻醉剂治疗难治性抑郁症安全、有效,且患者BDNF水平随抑郁程度好转而升高。
