颈动脉粥样硬化斑块的超声表现与微血管密度的相关性研究 被引量:9

Correlation between the different conventional ultrasonic performance and microvessei density in atherosclerotic plaques

摘要 目的探讨颈动脉粥样硬化的不同二维超声表现与微血管密度(MVD)的相关性。方法63例行颈动脉斑块剥离术的患者,共有82个斑块,测量斑块的大小、管径面积狭窄率及斑块厚度,根据斑块内部回声特性将斑块分为软斑(A组)、混合斑(B组)及硬斑(C组);术后对斑块行病理组织学检查及抗CD34免疫组化染色,计数MDV,分析其与斑块二维表现的相关性。结果①A组、B组及C组之间的MVD两两比较,差异具有统计学意义(P<O.05),且A组显著高于其他两组。②Pearson相关分析显示,A组随着斑块最大厚度及截面积狭窄率的增加,MVD呈上升趋势,且均为中度线性正相关(r=O.357,P=O.01;r=O.376,P=O.006);B组与C组,斑块最大厚度及截面积狭窄率与MVD之间不呈显著相关性(P>0.05)。结论颈动脉斑块的常规二维回声特征与MVD存在相关性,可反映斑块内的新生血管情况。 Objective To explore the correlation between the different conventional ultrasonic performance and microvessel density(MVD) of carotid plaques. Methods A total of 63 patients with 83 carotid atherosclerotic plaques were examined by ultrasound prior to undergoing a carotid endarterectomy (CEA).The maximum area,luminal area stenosis rate and the maximum thickness of plaque were recorded. According the echo of carotid plaques, three groups were divided into three groups: hypoechoic plaques (group A), mixed echoic plaques(group B),and hyperechoic plaques(group C).Then the plaque was get for pathological examination and CD34 monoclonal antibody immunohistochemical detection. The correlation between the performance and MVD was analyzed.Results OMVD was highest in the group A,next in the group B,lowest in the group C( P 〈0.05).②The following results were obtained by the Pearson correlation analysis.Group A,with the number of the maximum thickness and section area stenosis rate of the plaques increasing, MVD was heightened, there was moderate positive correlation between them ( r = 0.357, P = 0.01;r = 0.376, P = 0.006).Group B and C, there was no correlation between them. Conclusions The intraplaque neovascularization was correlation with the different conventional ultrasonic performance, such as the echo characteristics,the maximum thickness and section area stenosis rate of the atherosclerotic plaques.
