中重度宫腔粘连术后应用不同剂量雌激素对预后的影响 被引量:10

摘要 目的评价用不同剂量的雌激素(戊酸雌二醇)行人工周期对中、重度宫腔粘连(IUA)患者宫腔粘连电切术(TCRA)后临床疗效的影响。方法选择中、重度IUA的患者96例,行TCRA后随机分为两组,分别给予不同剂量戊酸雌二醇3 mg/d组或9 mg/d组并配合后半期孕激素(黄体酮胶囊)200 mg/d行人工周期治疗,均服用3周期。定期随访药物副反应及月经改善情况,术后3月宫腔镜检评价术后宫腔情况。结果中重度IUA患者术后辅以人工周期治疗,使用9 mg/d戊酸雌二醇组的治愈率、有效率均高于3 mg/d组,差异有统计学意义(P<0.05);且无严重不良反应。术后使用9 mg/d戊酸雌二醇,中度IUA患者的治愈率、有效率高于重度IUA患者,差异有统计学意义(P<0.05)。结论中重度IUA患者TCRA后行人工周期治疗,使用大剂量雌激素(9 mg/d)能提高TCRA的临床疗效,优于低雌激素(3 mg/d)组,且是安全的。中度IUA患者的预后要优于重度的。
