分期辨证治疗小儿过敏性紫癜的临床体会

Clinical Experience of Four Differential Treating Henoch—Schonlein Purpura in Children by Stages

过敏性紫癜是一种以广泛小血管炎为基础的系统性血管炎病变,主要累及皮肤、胃肠道、关节和肾脏。导师丁樱教授根据多年临床经验总结,认为小儿过敏性紫癜病机主要表现为"热、虚、瘀"三个方面,将过敏性紫癜根据病程特点分为初期、迁延期及后期三个阶段,在治疗上采用分期辨证法,即初期以祛邪安络为主,迁延期以扶正祛邪为主,后期加强健脾扶正,活血化瘀贯穿始终,在临床应用中不仅有效的缓解症状控制病情,还能减少肾脏损害的发生。

Henoch--Schonlein Purpura a kind of systemic vasculitis, which is based on generalized Small vessel vasculitis, can affect the skin, joints, gastrointestinal tract and kindeys. After painstaking research for decades, professor Ding Ying summarize pathogenesis that including three aspects were ＂ hot＂ ,＂ false＂ ,＂ stasis＂ , and divided the disease into three phases by the characteristics of course. They are early stage, protracted course, and later stage. In the treatment of Henoeh--Schonlein Purpura, the main emphasis of the early treatment is eliminating pathogen and removing obstruction in the meridians, strengthening healthy qi to eliminate pathogens is the emphasis in protracted course, and in the later stage strengthening spleen to support healthy qi is more importent, activating blood circulation to dissipate blood stasis is throughout the treatment. The clinic application show that, it not only relieves symptoms effectively, but also reduses the occurrence of renal damage.
