66例年轻女性乳腺癌临床及病理特点分析

The clinical and pathological characteristics of young female breast cancer: the analysis of 66 cases

目的:分析年轻女性乳腺癌的临床及病理特点。方法:选择66例≤35岁经病理确诊并采用免疫组化检测、手术治疗的乳腺癌患者作为研究,分析其临床及病理资料,并与随机抽取同期收治的35岁以上的中老年女性乳腺癌66例比较其差异。结果:年轻组在腋窝淋巴结转移率高于中老年组(P<0.05),病理分期III-IV期患者比例高于中老年组(P<0.05),PR阳性率低于中老年组(P<0.05),Her-2阳性率高于中老年组(P<0.05),选择保乳手术比例高于中老年组(P<0.05),两组在病程小于1个月比例、肿瘤直径大小、病理类型、ER阳性表达率比较,差异无统计学差异。结论:年轻乳腺癌就诊时病理分期晚、腋窝淋巴结转移率高,且较中老年组,年轻组PR阳性率低、Her-2高表达。

Objective To investigate the clinical and pathological characteristics of young breast cancer. Methods The young group choose 66 cases young breast cancer from 2009 to 2013 in our hospital who under 35 year old, which were diagnosed by immunohistochemical detection, treatmented by surgical, and analyze its clinical and pathological data, then select the same period to the hospital complaining of elderly patients with breast cancer 66 cases as the elderly group . Compare their differences of clinical and pathological characteristics between the two groups. Results Young group in axillary lymph node metastatic rate is higher than in elderly group （52% VS 30%, P 〈 0. 05）, pathological stage HI - IV a proportion of patients higher than the elderly group （48% VS 27%, P 〈 0. 05）, the PR positive rate is lower than the elderly group （35% VS 67%, P 〈 0. 05）, the Her -2 positive rate is higher than elderly group （74% VS 24%, P 〈 0. 05）, the choice of reservation mamma in operation ratio is higher than the elderly group （20% VS 3%, P 〈 0. 05）, Besides, there were no significant differences in the proportion of less than one month during the course, diameter of the tumor size, pathological type, ER positive expression rate. Conclusion Young breast cancer pathological staging was late, axillary lymph node metastasis rate is high, compared with elderly group , young group PR positive rate is low and its Her- 2 is high expression.
