摘要
目的:探讨妊娠期糖尿病患者胎盘组织中IRS-1和GLUT4的表达。方法选取2012年3月∽2014年3月本院产科门诊收治的住院分娩孕妇共83例,按照检查结果将其分为妊娠期糖尿病组(GDM组)(n=43)和正常足月妊娠组(对照组)(n=40)。检测两组孕妇的各临床指标(身高、血压、体重以及空腹血糖、空腹胰岛素、总胆固醇、三酰甘油等),同时采用逆转录-聚合酶链式反应(RT-PCR)方法测定患者胎盘组织中的IRS-1和GLUT4的表达水平。结果 GDM组的空腹血糖、空腹胰岛素、总胆固醇、三酰甘油等指标水平均高于对照组(P〈0.05)。 GDM组的HOMA-IR高于对照组,GDM组的HOMA-ISI、HOMA-β%低于对照组(P〈0.05)。IRS-1在两组胎盘组织中的表达水平比较,差异有统计学意义(P〈0.05);GLUT4在两组胎盘组织胞质中的表达水平的比较,差异无统计学意义(P〉0.05),在胎盘组织细胞膜中的表达水平比较,差异有统计学意义(P〈0.05)。结论 IRS-1、GLUT4作为胰岛素信号通路中的信号转导分子,其在胰岛素抵抗环节中发挥重要作用。
Objective To explore the Expression of IRS-1,GLUT4 in placenta of patients with gestational diabetes mellitus. Methods 83 pregnant women treated in outpatient department in ourhosptal from March 2012 to March 2014 obstetric outpatient were selected and the gestational diabetes mellitus (GDM) group (n=43)and the The normal full-term pregnancy group (the control group) (n=40) according to examination results.Different clinical indicators (height, blood pressure,weight,and fasting blood glucose, fasting insulin,total cholesterol,triglyceride,etc.), in the two groups were detected,expression level of IRS-1 and GLUT4 of placenta in patients were determined by reverse transcription poly-merase chain reaction (RT-PCR) methods in the two groups. Results The level of fasting glucose,fasting insulin,total cholesterol,and triglycerides and so on in the GDM group were higher than those in the control group (P〈0.05).HOMA-IR of the GDM group was higher than that in the control group,HOMA-ISI,HOMA-β% the GDM group was lower than that in the control group (P〈0.05).There was a statistical difference of IRS-1 expression level of placenta in the two groups (P〈0.05);there was no statistical difference of GLUT4 expression level of cytoplasm in placenta of the two groups (P〉0.05),there was a statistical difference of GLUT4 expression level of Cell membrane in placenta of the two groups (P〈0.05). Conclusion As a signal transduction molecule in the insulin signaling pathway,IRS-1,GLUT4 can play an im-portant role in insulin resistance.
