摘要
目的了解漳州地区女性宫颈病变人乳头瘤病毒(HPV)感染及HPV基因型分布。方法采用等离子表面振谐技术对2012年1月—2014年1月解放军175医院宫颈活检常规诊断见到异常细胞(疑似尖锐湿疣、CIN、宫颈癌)的200例标本进行HPV基因分型检测。结果 200例中HPV阳性84例,总阳性率为42.0%,单一病毒感染占感染例数的79.8%(67/84),双重(13例)及多重(4例)感染占感染例数的20.2%(17/84);高危型阳性占总例数的38.0%(76/200),占感染例数的90.5%;低危型阳性占总例数的15.0%(30/200),占感染例数的35.7%。高危型阳性以HPV16型比例最高,占高危型感染的39.5%(30/76),余比例较高的型别由高到低依次为HPV81、59、52、53、58和18型;低危型阳性以HPV11型比例最高,占低危型感染的50.0%(15/30),其次为HPV54、6型。结论漳州地区女性宫颈病变HPV单一、双重及多重感染并存,基因型分布较广,但主要感染基因型高危以HPV16、81、52型为主,低危以HPV11、54、6为主,应针对高发感染的HPV基因亚型进行宫颈病变的防治。
Objective To study the infection and distribution of human papilloma virus (HPV) genotypes in cervical lesions of female patients in Zhangzhou. Methods HPV genotypes of 200 patients, who had abnormal cervical cells ( suspected condyloma acuminatum, CIN and cervical cancer) by cervical biopsy during January 2012 and January 2014, were detected with the method of surface plasmon resonance ( SPR) technique. Results A total of 84 positive HPV cases were detected among the 200 cases, and the total positive rate was 42. 0%, the rate of single genotype infec-tion to infection cases of HPV was 79. 8% (67/84), while the rate of double (13 cases) and multiple (4 cases) infec-tions to infection cases was 20. 2% (17/84);the rate of high risk of positive subtype and low risk of positive subtype a-mong the total cases were 38. 0% (76/200) and 15. 0% (30/200) respectively, and the rate to infection cases were 90. 5% and 35. 7% respectively;HPV16 was the most frequent high risk of positive subtype, and the rate to all high risk positive subtypes was 39. 5% (30/76), and then from high to low, the higher subtypes were followed by HPV81, HPV59, HPV52, HPV53, HPV58 and HPV18;HPV11 was the most frequent low risk of positive subtype, and the rate to all low risk of positive subtypes was 50. 0% (15/30), and then followed by HPV54 and HPV6. Conclusion The in-fected HPV genotype of cervical lesions in women is common in Zhangzhou area with single, double and multiple infec-tions, but the main high risk of infection subtypes are HPV16, HPV81 and HPV52, and the main low risk of subtypes are HPV11, HPV54 and HPV6. The prevention and treatment of cervical lesions should be performed according to the high incidence of HPV infection subtypes.
