非营养性吸吮在新生儿动脉静脉采血中的应用

目的探讨新生儿动脉、静脉穿刺采血时给予非营养性吸吮是否可减轻疼痛,提高穿刺成功率以及采血后缩短按压时间的方法。方法将我院新生儿科2010—2011年住院患儿胎龄>35周的新生儿100例随机分为干预组和对照组各50例,干预组在进行穿刺采血时给予非营养性吸吮,对照组在采血时不给予任何干预。采用美国国际圣路加医疗中心NICU评估量表进行疼痛的评估,观察两组患儿在穿刺采血时的疼痛反应,并对两组新生儿采血后止血效果进行比较。结果干预组患儿在采血时穿刺成功率高,止血按压时间短,采血局部皮肤渗血、瘀斑及皮下血肿的发生率降低。对照组在采血时新生儿所表现的状态比干预组紧张,止血按压时间长。两组比较出血情况,干预组有效率为96%;对照组有效率为76%。疼痛评分干预组和对照组比较差异有统计学意义(P<0.01)。结论新生儿动、静脉穿刺采血时进行非营养性吸吮可减轻医源性刺激时的疼痛反应,提高穿刺采血的成功率,缩短采血后按压时间,有利于止血。

OBJECTIVE To investigate the neonate artery, vein puncture blood to give non nutritional sucking on how to reduce pain and improve the success rate of puncture and shorten after blood pressure time method. METHODS 2010---2011 hospitalized children with gestational age beyond 100 cases with neonatal 35 weeks each 50 cases were randomly divided into intervention group and con- trol group, intervention group in the puncture blood of nonnutritive sucking, control group not given any intervention in the blood. NICU in U.S. international st Luke＇s medical center assessment scale for assessment of pain, and observing two groups of children in the piercing pain response to blood, and the two groups of neonatal blood hemostasis effect after comparison. RESULTS Intervention group of children in the blood when the puncture success rate was high, the bleeding press time was short, bruises, local skin blood oozing, reduce the incidence of subcutaneous haematomas. The control group in blood of newborn state intervention group, the bleeding on long time. Comparison of bleeding in both groups, intervention group effective rate was 96%. The control group effective rate was 76%. Pain score intervention group and control group with significant difference comparing statistical significance （P〈0.01）. CONCLUSION Newborn, when venipuncture in pursuance of nonnutritive sucking could ease the pain of iatrogenic stimulation reaction, improve the success rate of the puncture blood after blood pressure time, which was good for stoping the bleeding.
