奥沙利铂用于胃癌术后早期腹腔热灌注化疗的临床观察

Clinical study of early postoperative chemotherapeutic hyperthermic intraperitoneal perfusion with oxaliplatin for gastric cancer

目的 观察进展期胃癌根治术后早期给予奥沙利铂（L-OHP）行腹腔热灌注化疗（CHIP）的不良反应及疗效.方法 将83例进展期胃癌随机分为CHIP组和对照组,应用SPSS13.0分析并发症、复发率、转移率和无瘤生存率.CHIP组共37例,术后第1天给予L-OHP 100 mg/m2行CHIP,1次/d,4次为1疗程.对照组46例给予常规处理.结果 两组严重并发症发生率差异无统计学意义.CHIP组的2～3年无瘤生存率明显高于对照组,差异有统计学意义.结论 进展期胃癌术后早期给予L-OHP行CHIP,对提高2～3年无瘤生存率有明显疗效,而且方法简单、安全、实用.

Objective To study the efficacy of early postoperative chemotherapeutic hyperthermic intraperitoneal perfusion( CHIP) with oxaliplatin( L- OHP) with advanced gastric cancer. Methods From January 2009 to December 2010,83 patients with advanced gastric cancer were admitted. They were divided into CHIP group and control group. The complication,celiac recurrence rate,liver metastasis rate and disease- free survival rate were followed up. SPSS 13. 0 software was used. Thirty- seven of the 83 patients,beginning from the first day of postoperative,were given CHIP,once per day for 4 days,with warm( 44- 45℃) 5% glucose containing 100 mg /m2 L- OHP. The other 46 patients,as control,were given routine treatment. Results There was no significant difference in the postoperative serious complications between the two groups. In patients treated with CHIP,the 2- year and 3- year disease- free survival rates were 87. 2% and 77. 4%,and the control's rates were 64. 0% and 52. 3%. There were significant differences between the two groups. Conclusion Early postoperative CHIP with L-OHP is helpful in improving the 2- year and 3- year disease- free survival rates of advanced gastric cancer. The treatment procedure is safety and could be tolerated.
