疝囊高位结扎和脐内侧襞加强耻骨肌孔在儿童腹股沟复发疝腹腔镜修补术中的应用

Application of laparoscopic high ligation and strengthened myopectineal orifice in the treatment of pediatric recurrent inguinal hernia

目的探讨疝囊高位结扎和脐内侧襞加强耻骨肌孔在儿童腹股沟复发疝腹腔镜修补术中的作用。方法回顾性分析28例接受疝囊高位结扎和脐内侧襞加强耻骨肌孔治疗小儿腹股沟复发疝的患者资料。结果 25例完成单侧手术,2例完成双侧手术;1例脐正中襞和左右脐内侧襞较窄,不能实施原计划手术,行内环口高位双重结扎。术后1例患侧阴囊气肿,持续5 d后消失;1例患侧腹股沟区牵拉疼痛不适感,口服可缓解,持续12 d逐渐消失。结论该手术是治疗大龄儿腹股沟复发疝的新方法,但有长时间的腹股沟区牵拉疼痛不适感,远期效果有待更长时间的随访和大宗病例的治疗效果。

Objective To explore the application of laparoscopic high ligation and strengthened myopectineal orifice in the treatment of pediatric recurrent inguinal hernia. Methods The clinical data of twenty eight cases were collected for a retrospective analysis. Results Twenty-five cases finished a b/lateral operation and 2 unilateral operations. The other one underwent inner high ligation due to the navel narrowness. One patient had postoperative scrotum swelling and disappeared five days later, while one patient suffered from pulling and pain in the inguinal area and also disappeared 12 days later because of taking analgesics. Conclusions It is a new technique for treating pediatric recurrent inguinal hernia, hut needs further long-term follow-up and muhicenter analysis for the complications of inguinal pulling and pains.
