双相Ⅰ型与双相Ⅱ型患者认知功能损害的对比研究 被引量:2

A study on cognitive impairment of the bipolar Ⅰ and bipolar Ⅱ patients

摘要 目的探讨双相I型障碍与双相II型障碍患者认知功能损害的特点及差异。方法收集51例双相I型障碍患者(A组)、51例双相II型障碍患者(B组)与51例正常对照者(C组),采用持续操作测验(CPT)和威斯康星卡片分类测验(WCST)评估与比较其认知功能。结果 (1)CPT测验情况:A组、B组的正确数均显著低于C组,A组、B组的错误数均显著高于C组,A组的正确数显著低于B组,而错误数显著高于B组(均为P<0.05);(2)WCST测验情况:A组、B组的错误应答数、非持续性错误数均显著高于C组,而正确应答百分比显著低于C组,A组与C组相比除在上述方面有显著差异外,A组在总应答数、完成分类数均显著差于C组,而不能维持完整分类数则显著高于C组(均为P<0.05);A组的完成分类数显著低于B组,A组的总应答数、错误应答数、持续性应答数、非持续性错误、不能维持完整分类数、学习到学会方面均显著高于B组(均为P<0.05)。结论双相I型障碍与双相II型障碍患者均存在多维度的认知功能损害,双相I型障碍患者在注意障碍、认知转移能力等方面受损的程度重于双相Ⅱ型障碍患者。 Objective To investigate the characteristics and the relationship of cognitive impairment between the bipolar Ⅰ and bipolar Ⅱ patients. Methods collected 51 patients with bipolar Ⅰ disorder( A group),51 patients with bipolar Ⅱ disorder( B group) and 51 normal controls( C group),who were assessed with the continuous performance test( CPT) and the Wisconsin Card Sorting Test( WCST) to evaluate and compare their cognitive function. Results( 1) The CPT: There are a significant differences in three groups in the correct number and the error number of the CPT( P〈0. 05);( 2) The WCST: Three groups showe significant difference in the WCST’s error response number,correct answer percentage,non-persistent errors( P〈0. 05); group A and group C have also significant differences in the number of overall response,complete the number of categories of the WCST( P〈0. 05); group A and group B show a significant difference in the correct number,the number of errors of CPT and the total answer number,the completion of a number of categories,non-persistent error,the number of sustained response,error response number,maintain the full number of categories of the WCST( P〈0. 05). Conclusion( 1) The bipolar Ⅰ and bipolar Ⅱ patients have cognitive impairments in multiple dimension.( 2) Bipolar Ⅰ patients are characterized by more severe cognitive impairment relative to bipolar Ⅱ patients.
