宫颈上皮病变组织中LIN28B表达与高危型HPV感染的关系及临床意义 被引量:6

The role and clinical significance of LIN28B expression in HR-HPV infection

摘要 目的探讨LIN28B在宫颈上皮病变组织中表达与高危型人乳头状瘤病毒(HR-HPV)感染的关系及临床意义。方法应用免疫组化方法检测102例宫颈癌、83例宫颈上皮内瘤变(CIN)及33例宫颈炎组织中LIN28B表达情况,同时于术前采用第二代基因杂交捕获法(HC-Ⅱ)检测患者HR-HPV感染情况。对随访3年以上宫颈癌患者进行生存分析。结果 LIN28B在宫颈癌中高表达率均高于CIN和宫颈炎(P<0.05)。LIN28B表达与组织学分级、FIGO分期、浸润深度及淋巴结是否转移有关(P<0.05)。HR-HPV在宫颈炎、CIN和宫颈癌中感染率依次升高(P<0.05),但与临床病理参数均无关(P>0.05)。LIN28B表达与HR-HPV感染呈正相关(P<0.05)。LIN28B表达与预后有关(P<0.05),而HR-HPV感染与预后无关(P>0.05)。结论 LIN28B表达和HR-HPV感染可反映宫颈上皮病变的程度,联合检测二者有助于宫颈癌筛查及临床病理因素的判断,LIN28B表达与宫颈癌患者的预后密切相关。 Objective To investigate the expression of LIN28 B in cervical intraepithelial lesions and its role and clinical significance in HR-HPV infection. Methods The expression of LIN28 B was detected in 102 cases of cervical carcinoma,83 cases of cervical intraepithelial neoplasia(CIN) and 33 cases of chronic cervicitis by immunohistochemical method,and HR-HPV infection was detected by HC-Ⅱ(hybrid captureⅡ) before operations. The survival analysis was performed on cervical carcinoma patients after follow-up more than three years. Results The expression levels of LIN28 B were higher in cervical carcinoma than those in CIN and chronic cervicitis(P〈0. 05). The LIN28 B expression was associated with histological grading,FIGO stage,infiltration depth and lymphode metastasis(P〈0. 05). The positive rates of HR-HPV infection increased gradually in chronic cervicitis,CIN and cervical carcinoma(P〈0. 05),but not correlated with clinicopathologic parameters(P〉0. 05).The expression of LIN28 B was positively correlated with HR-HPV infection(P〈0. 05),and correlated with prognosis(P〈0. 05),but HR-HPV infection was not correlated with prognosis(P〉0. 05). Conclusion LIN28 B expression and HR-HPV infection can reflect the degree of cervical intraepithelial lesions,and combined detection contribute to cervical carcinoma screening and evaluation of clinical pathological factors. The expression of LIN28 B is closely related with prognosis of patients with cervical carcinoma.
