血清胃蛋白酶原及肿瘤标志物对胃癌的诊断价值

Diagnostic value of serum pepsinogen and tumor markers for gastric cancer

目的探讨血清胃蛋白酶原及肿瘤标志物对胃癌的诊断价值。方法选取2013年1月-2013年10月经胃镜活检病理诊断胃癌40例为实验组,健康体检者及浅表性胃炎、十二指肠溃疡等良性胃病共40例为对照组。晨起空腹采静脉血,定量检测血清中胃蛋白酶原水平(PG-Ⅰ、PG-Ⅱ、PG-Ⅰ/PG-Ⅱ)及检测血清癌胚抗原(CEA)、糖蛋白抗原199(CA199)、糖蛋白抗原72-4(CA72-4)等肿瘤标志物的水平,比较二组间的差异。结果胃癌患者血清中各标志物检测水平与对照组比较差异有统计学意义(P<0.05)。肿瘤标志物联合检测的敏感性为82.1%与单项检测相比差异有统计学意义(P<0.05)。结论 PG-Ⅰ、PG-Ⅰ/PG-Ⅱ、CEA、CA199、CA72-4对胃癌具有较高的辅助诊断价值,其联合检测有助于提高胃癌阳性检测率。

Objective To investigate the diagnostic value of joint detection of serum pepsinogen and tumor markers for gastric cancer. Methods Forty gastric cancer cases as the experimental group and 40 cases consisting of healthy persons,superficial gastritis patients,stomach ulcers patients and other benign gastric diseases patients as the control group were selected to detect the levels of pepsinogen（PG-Ⅰ and PG-Ⅰ/PG-Ⅱ）,carcino embryonie antigen（CEA）,carbohydrate antigen 199（CA199）,carbohydrate 72-4（CA 72-4） in fasting blood. The obtained data were compared between the two groups. Results The serum levels of five tumor markers（CEA,CA199,CA 72-4,PG-Ⅰand PG-Ⅰ/PG-Ⅱ） in experimental group were significantly different from those in the control group（P〈0. 05）. The sensitivity of the joint detection of the five tumor markers（82. 1%） was significantly higher than that of the respective detection（P〈0. 05）. Conclusion The levels of serum CEA,CA 199,CA 72-4,PG-Ⅰand PG-Ⅰ/PG-Ⅱare very valuable in diagnosis of gastric cancer. The joint detection helps to improve the positive detection rate of gastric cancer.
