Evaluation and Analysis on 12 230 Cases of Outpatients Western Medicine Prescription in Hospital

目的:了解门诊处方用药情况,促进临床合理用药。方法:抽取本院2011年1月-2013年12月的门诊处方共12 230张,按照《医院处方点评管理规范》及《处方管理办法》要求进行处方评价。结果:本院处方合格率较低,为76.27%;国家基本药物占处方用药的比例较高,为90.83%;药品通用名使用率较高(98.24%);平均每张处方用药品种控制较好(1.56种)。不合理处方主要为不规范处方,占不合理处方的22.22%,不规范处方主要表现在处方超7 d用量未注明理由(6.84%)及处方缺项、字迹潦草(5.81%),其他为抗菌药物使用不合理(2.91%);用法用量不详细不规范(2.21%)等。结论:本院应完善处方点评工作制度,加强处方质量和药物临床应用管理,规范医师处方行为,促进合理用药。

Objective： To investigate the situation of drug usage in the outpatient prescription and to promote rational use of drugs. Method： 12 230 outpatient western medicine prescriptions from Januray 2011 to December 2013 in our hospital were collected and evaluated in accordance with ＂Hospital Prescription Requirement for Evaluation＂ and ＂Prescription Management＂ .Result： Qualification rate of prescriptions in our hospital was very low and it was 76.27%; national essential drugs account for a higher proportion of prescription drugs and was 90.83%; the utilization rate of drug generic name was higher and it was 98.24%; drug varieties in the average prescription was well controlled （ 1.56 ） . Unreasonable prescriptions were mainly nonstandard ones which account for 22.22% of the unreasonable prescriptions. Nonstandard prescriptions are characterized by super 7 dosage without clear reason （ 6.84% ）, page missing, poor handwriting（ 5.81% ）,irrational use of anti-bacteria drugs（ 2.91% ）;undefined and nonstandard usage and dosage（ 2.21% ） etc.Conclusion： It is necessage to improve the work system of prescription reviews, enhance the quality of prescription and management of drugs clinical application and standardize prescribing behavior of physicians in order to promote rational use of drugs.
