医用生物胶体液及愈合膜在鼻内镜手术中的应用研究

Study on Application of Medical Biological Colloid and Healing Membrane in Nasal Endoscopy Operation

目的:探讨鼻内镜手术中应用医用生物胶体液及愈合膜的临床效果。方法:将鼻内镜手术62侧(40例患者)按照随机数字表法分为试验组32侧和对照组30侧,试验组应用医用生物胶体液灌洗术腔,再用愈合膜涂抹在脑棉片上,填入术腔;对照组用膨胀海绵加压填塞术腔。观察两组患者术后的止血效果、疼痛反应及抽出填塞物后鼻腔黏膜再次出血、鼻腔粘连、术后鼻腔换药次数、鼻腔通气功能恢复时间等指标。结果:两组术后止血效果满意,差异无统计学意义(P>0.05);试验组的鼻腔胀痛发生率、鼻腔黏膜再出血率和鼻腔粘连率均明显少于对照组,差异均有统计学意义(P<0.01或P<0.05)。试验组术后平均术腔换药次数明显少于对照组,且平均鼻腔恢复正常通气时间短于对照组,差异均有统计学意义(P<0.05)。结论:鼻内镜手术中应用医用生物胶体液及愈合膜,止血效果好,术后鼻腔胀痛少,术后粘连少,术后换药次数少,鼻腔通气功能恢复快,值得临床推广应用。

Objective： To investigate the clinical effect of nasal endoscopic operation in the application of medical biological colloid and healing of membrane.Method ： 62 sides of nasal endoscopic operation （ 40 patients ） according to the random number table method were randomly divided into the experimental group for 32 sides and the control group for 30 sides.The experimental group was used the medical biological colloid to lavage the solution cavity, and then the healing membrane coated on cotton pads was filled in the nasal cavity.In the control group, the expansive sponge was filled in the operation cavity.The hemostatic effect, postoperative pain reaction and extraction stuffing nasal mucosa after the rebleeding, nasal cavity adhesion, postoperative ventilation of nasal cavity nasal dressing times, recovery of normal nasal ventilation time and other indicators of the two groups were observed and compared.Result： The hemostatic effect of two groups were satisfied after the operation, there was no significant difference between the two groups （ P〉0.05 ） . The incidence of nasal pain, nasal mucosal bleeding rate and nasal cavity adhesion rate in experimental group were significantly lower than those in the control group, the differences were statistically significant （ P〈0.01 or P〈0.05 ） . The average postoperative surgical cavity dressing change times of the experimental group was significantly less than the control group, the average recovery of normal nasal ventilation time was significantly shorter than the control group, the differences were statistically significant （P〈0.05） .Conclusion： In the endoscopic operation, the application of medical biological colloid and healing of the film have a good hemostatic effect after operation, nasal pain less, adhesion, dressing change times less, nasal ventilation function recovery, is worthy of clinical application.
