宫腔镜子宫内膜去除术的临床应用

Clinical evaluation of clinical of endometrial ablation

目的：探讨宫腔镜子宫内膜去除术（EA）治疗异常子宫出血（AUB）的手术指征及疗效。方法：对2010年4月至2012年8月行EA的36例临床资料进行回顾性分析,比较手术前后月经量、血Hb水平,术后6个月刮宫观察子宫组织的病理学改变。结果：术后随访10~30个月,月经改善总有效率为97.2%（35/36）,其中闭经36.1%（13/36）,月经量减少61.1%（22/36）;术后贫血纠正率为94.4%（34/36）。结论：EA是治疗异常子宫出血安全有效的办法,尤其适用于有严重内科合并症的AUB患者,是替代子宫切除的一种治疗方法。

Objective To explore the indications of endometrial ablation（EA）and its efficacy for abnormal uterine bleeding（AUB）.Methods A retrospective analysis was made of 36 patients with AUB,who received EA from August2010 to April 2012.We compared the rate of menstrual volume and serum levels of Hgb with those before the surgery.All the patients underwent dilatation and curettage 6months after treatment for histopathological examination of endometrium.Results The follow-up period after operation was 12 to 30months.Overall effective rate was 97.2%,in which amenorrhea rate was 36.1%,menstruation reduction rate was 61.1%.The effective rate of corrective anemia was 94.4%.Conclusions EA is a safe and effective alternative treatment for AUB,especially suitable for AUB women with severe medical complications.It＇s a treatment substitute for hysterectomy.
