两种三联疗法根除幽门螺杆菌感染胃溃疡临床疗效比较

Comparison between clinical effects of two kinds of triple therapy for the eradication of helicobacter pylori infection in gastric ulcer

目的对两种三联疗法治疗幽门螺杆菌感染(HP)胃溃疡的效果进行比较和探讨。方法 46例幽门螺杆菌相关性胃溃疡患者分为对照组和观察组,每组23例。对照组应用奥美拉唑、阿莫西林与甲硝唑进行治疗,观察组应用奥美拉唑、阿莫西林与克拉霉素进行治疗,对采用不同三联疗法治疗的患者的临床疗效及幽门螺杆菌根除率进行比较。结果经治疗4周后,对照组有4例治愈,12例有效,7例无效,总有效率为69.56%;观察组有9例治愈,13例有效,1例无效,总有效率为95.65%,组间比较差异具有统计学意义(P<0.05)。对照组HP根除率为60.87%(14/23),观察组HP根除率为86.97%(20/23),差异具有统计学意义(P<0.05)。结论应用奥美拉唑+阿莫西林+克拉霉素三联疗法治疗幽门螺杆菌相关性胃溃疡,其治疗效果好于奥美拉唑+阿莫西林+甲硝唑,可提高HP根除率,促进溃疡面愈合,值得临床借鉴与推广。

Objective To compare and discuss the clinical effects of two kinds of triple therapy for the eradication of helicobacter pylori infection （HP） in gastric ulcer.Methods A total of 46 cases of helicobacter pylori-associated gastric ulcer patients were divided into control group and observation group, and each group contained 23 cases. Omeprazole, amoxicillin and metronidazole were used in the control group for treatment, while omeprazole, amoxicillin and clarithromycin were applied in the observation group. Comparison was made between the clinical effects and helicobacter pylori bacterial eradication rates of patients treated by different triple therapy.Results After 4 weeks of treatment, there were 4 cured cases, 12 effective cases and 7 ineffective cases in the control group, and the total effective rate was 69.56%. In the observation group, there were 9 cured cases, 13 effective cases and 1 ineffective case, and the total effective rate was 95.65 %. The difference between the two groups was statistically significant （P〈0.05）. The HP eradication rate of the control group was 60.87% （14/23）, and that of the observation group was 86.97% （20/23）. The difference had statistical significance （P〈0.05）. Conclusion In treating the helicobacter pylori-associated gastric ulcer, the triple therapy using omeprazole, amoxicillin, and clarithromycin has better clinical effect than the application of omeprazole, amoxicillin, and metronidazole. The former can improve HP eradication rate and promote healing of ulcers, which is worthy of clinical reference and promotion.
