3D 补片与平片补片在治疗腹股沟疝的临床疗效对比

Comparison of Clinical Efficacy of 3D Patch vs .Plain Film Patch for Repairing Inguinal Hernia

【目的】对比分析使用3D 补片和平片补片经腹腔腹膜前疝气修补术治疗腹股沟疝的临床疗效。【方法】回顾性分析本院自2011年8月至2013年8月行经腹腔腹膜前疝气修补术113例患者临床资料。其中56例患者采用3D补片修补（观察组），57例采用平片补片修补（对照组）；观察组无须固定，对照组采用金属钉固定。比较两组患者的手术时间，疼痛评分，复发例数以及术后并发症的发生情况。【结果】观察组和对照组手术时间和住院天数两组之间相比较差异均无显著性（P ＞0．05）；观察组术后d1，d7的疼痛评分分别为1．5±0．3，0．4±0．5，对照组为2．6±0．7和1．1±0．4，两者相比较差异均具有显著性（P ＜0．05）；观察组复发率为1．79％（1/56），对照组为7．02％（4/57），两者相比较差异具有显著性（ P ＜0．05）。观察组切口感染的发生率为5．36％（3/56），对照组为7．02％（4/57），两组相比较差异无显著性（ P ＞0．05）。观察组阴囊血肿的发生率为1．79％（1/56），对照组为10．53％（6/57），两者相比较差异具有显著性（ P ＜0．05）。【结论】行3D补片经腹腔腹膜前疝气修补术治疗腹股沟疝的临床疗效优于平片补片，值得临床推广应用。

Objective To comparatively analyze the clinical efficacy of 3D patch vs .plain film patch via abdominal preperitoneal hernia repair for the treatment of inguinal hernia .[Methods] Totally 113 patients undergo-ing abdominal preperitoneal hernia repair in our hospital from Aug .2011 to Aug .2013 were collected .The observation group（ n=56） underwent 3D patch repair without fixation ,while the control group（ n = 57） underwent plain film repair with metal nail fixation .The operation time ,pain score ,recurrent case and the incidence of postoperative complications were compared between two groups .[Results] There was no significant difference in operation time and hospital day between the observation group and control group （ P〉0 .05） .The pain scores in observation group at d1 and d2 after operation were 1 .5 ± 0 .3 and 0 .4 ± 0 .5 respectively ,and those in control group were 2 .6 ± 0 .7 and 1 .1 ± 0 .4 respectively ,and there was significant difference between two groups （ P 〈0 .05） . The recurrence rates of observation group and control group were 1 .79% （1/56） and 7 .02% （4/57） respectively , and there was significant difference between two groups（ P〈0 .05） .The incidence rates of wound infection in observation group and control group were 5 .36% （3/56） and 7 .02% （4/57） ,and there was no significant difference between two groups（ P 〉0 .05） .The incidence rates of scrotal hematoma in observation group and control group were 1 .79% （1/56） and 10 .53% （6/57） respectively ,and there was significant difference（ P 〈0 .05） .[Conclusion]Clinical efficacy of 3D patch via abdominal preperitoneal hernia repair for the treatment of inguinal hernia is better than plain film patch .The efficacy is certain .
