艰难中前行的奥巴马政府医疗改革被引量：4

摘要本文在分析美国医疗改革背景和医疗系统概况的基础上,阐述2013年以来奥巴马政府医疗改革主要进展及所遇到问题,并对2014年美国医疗改革趋势进行探讨。2013年以来,奥巴马政府的医疗改革取得了很多进展:创建了联邦、州或联合医疗保险交易所(或医疗保险市场);规范管理医疗保险公司;创新性改革公共医疗保险项目支付制度;定期公布多方位、透明医疗数据等将从根本上重塑美国医疗系统。但改革也遭遇各种问题:责任与权利、政府与市场作用争论激化;联邦、州医疗保险交易所设计及技术障碍;联邦、州政府面临财政预算压力;美国各州医保覆盖率和医保保费差距拉大。这些问题的存在将为2014年奥巴马医疗政策的执行增加不确定性。
