仙方活命饮辅助治疗妇科术后感染疗效及作用机制分析

Analysis of Effect and Mechanism of Infections after Gynecological Operation by Adjuvant Therapy with Xianfanghuoming Decoction

目的:分析仙方活命饮辅助治疗妇科术后感染疗效和作用机制,为临床预防和控制术后感染提供参考依据。方法:将300例妇科择期手术患者按照随机数字表法分为观察组和对照组各150例,对照组给予常规围术期用药预防术后感染,观察组在对照组治疗的基础上加用仙方活命饮。比较两组患者的术后感染发生率、血清指标和免疫功能变化情况。结果:观察组术后感染发生率为0.67%,高于对照组的4.67%,两组比较差异有统计学意义(P<0.05);观察组住院时间、开始进食时间、下床活动时间、术后用药费用均低于对照组,差异有统计学意义(P<0.05);术后两组CD+3、CD+4、CD+4/CD+8均有显著改善(P<0.05);术后3 d、术后5 d观察组CD+3、CD+4、CD+4/CD+8高于对照组,CD+8低于对照组,差异均有统计学意义(P<0.05);两组患者术后3 d、术后5 d的PCT、CRP、WBC水平均低于术前(P<0.05),观察组下降水平优于对照组,差异均有统计学意义(P<0.05)。结论:仙方活命饮应用妇科手术患者,能够有效降低术后感染发生率,改善患者的血清指标和免疫功能,缩短了患者恢复时间,值得临床推广应用。

Objective ： To investigate the Xianfanghuoming auxiliary treatment effect and the mechanism of infection, to provide reference for clinical prevention and control of postoperative infection. Methods ：300 cases of gynecologic operation were randomly divided into observation group and control group with 150 cases in each group. The control group received conventional perioperarive medication to prevent postoperative infection. The observation group added Xianfanghuoming Decoction. The postoperative infection rate, changes and immune serum were compared in two groups. Results ： The incidence rate was 0.67% of infected patients in the observation group, higher than that of the control group 4.67%. Two groups had significant difference （P 〈 0.05 ）. The observation group＇s hospitalization time ,feeding time, bed time, postoperative medication costs were lower than those of the control group. The difference was statistically significant （ P 〈 0.05 ）. After the surgery, two group＇s CD3＋ , CD4＋ , CD4＋/CDs were significantly improved （P 〈 0.05） ; 3d,5d after the operation, observation group＇s CD3＋ , CD4 ,CD4/CD8＋ were higher than that of the control group and CD8 was lower than that in the control group. The differences were statistically significant （P 〈0.05 ）. 3d,5dafter surgery the PCT, CRP, WBC levels were lower than that before operation （ P 〈 0.05 ）. The observation group decreased levels were superior to those of the control group. The differences were statistically significant （P 〈 0. 05 ）. Conclusion ： In patients with gynecological operation ,Xianfanghuoming Decoction can effectively reduce the postoperative infection rate, improve the indexes of serum and immune function of patients, shorten the time of recovery of patients. It is worthy of clinical application.
