乳腺癌根治术围手术期抗菌药物应用分析

Analysis on Utilization of Antimicrobials for Radical Mastectomy in Perioperative Period

目的了解本院2013年乳腺癌根治术围术期抗菌药物的应用,为加强抗菌药物的合理使用和规范管理提供依据。方法回顾性分析本院2013-01~11肿瘤外科行乳腺癌根治术患者出院病历191份,进行高危因素和围术期抗菌药物应用统计和分析。结果 191例乳腺癌手术患者中,17.80%的患者存在高危因素;抗菌药物使用率为14.7%;25例选用头孢呋辛,合理率为89.29%。术前给药时机正确率为32.1%;在3例手术时间超3 h的患者中,无一例于术中追加抗菌药物;围手术期抗菌药物未见联合用药;14例用药疗程超48 h。结论乳腺癌根治术围手术期预防使用率达到卫生部低于30%的要求。预防用抗菌药物品种选择基本合理;预防给药时机和疗程存在一定不足,应进一步加强管理。

Objective To evaluate the utilization of antimicrobials for radical mastectomy in perioperative period. Methods Medical records of patients underwent radical mastectomy were analyzed statistically and retrospectively. Results 34 patients had risk factors, and 55.88% of them were not given antimicrobials. 28 patients received antibacterial drugs preventively in perioperative period, 89.29% of which used cefuroxime for prophylaxis. Positive rate of administration time was 32.1%. 3 patients underwent operations over 3 h received no additional antimicrobial drugs in surgery. No antimicrobial drug combination was given in perioperative period. 14 patients received antimicrobials perioperative treatment more than 48 h.Conclusion The utilization of antibacterials in patients with radical mastectomy in perioperative period is appropriate on the whole, while there still are some problems.
