幼儿不稳定型骨盆骨折的手术治疗 被引量：6

摘要 目的 探讨幼儿不稳定型骨盆骨折的临床特点与手术治疗. 方法 回顾性分析2010年7月至2012年12月收治的2例幼儿不稳定型骨盆骨折患者资料,男1例,女1例;年龄分别为13、16个月;致伤原因均为汽车碾压伤.损伤情况：1例女性患儿右髂骨骨折、右耻骨上下支骨折、左骶髂关节脱位,伴有失血性休克和右股骨转子间骨折;另1例男性患儿耻骨联合分离,左髂骨骨折,伴有左股骨干骨折.2例患儿骨盆骨折按Torode-Zieg分型均为Ⅳ型.受伤至手术时间分别为5、18d.均采用切开复位内固定和外固定支架治疗. 结果 本组2例患儿手术时间分别为300、360 min,术中出血量分别为300、400 mL.2例患儿术后分别获48、19个月随访.术后骨折复位质量根据Matta标准评定：2例均为优.2例患儿骨折愈合时间分别为10、12周.末次随访时Majeed评分分别为91、95分,疗效评定均为优.末次随访时2例患儿双下肢均等长,行走无疼痛,下肢无旋转,步态良好. 结论 儿童骨盆骨折临床并不多见,＜2岁的幼儿更是极其罕见.对于移位明显且伴有耻骨联合分离及骶髂关节脱位的幼儿骨盆骨折患者,尽早行手术治疗可获得满意疗效.
