乳突筛区骨膜下注射甲泼尼龙琥珀酸钠治疗常规治疗失败的突发性聋

Clinical effect of methyllprednisolone on refractory sudden hearing loss through mastoid cribriform area subperiosteum injection

目的探讨经乳突筛区骨膜下注射甲泼尼龙琥珀酸钠(甲强龙)治疗常规治疗失败的突发性聋的疗效。方法经常规治疗30 d以上无效的突发性聋72耳,分为乳突筛区骨膜下注射给药组和对照组;乳突筛区骨膜下注射给药组36耳,对照组36耳。经乳突筛区骨膜下注射甲泼尼龙琥珀酸钠的治疗方法即从耳后乳突筛区骨膜下注射甲泼尼龙琥珀酸钠40 mg,隔日注射1次,7次1个疗程。对照组给药口服银杏叶片和甲钴胺片2周。所有患者在治疗后1周复查电测听。结果乳突筛区骨膜下注射给药组听力恢复12例耳有效,有效率33.30%,对照组无1例有效。二者差异有显著性统计学意义(χ2=14.40,P<0.05)。结论经耳后乳突筛区骨膜下注射甲泼尼龙琥珀酸钠治疗常规治疗失败的突发性聋仍可取得一定疗效。

OBJECTIVE To investigate the therapeutic effect of methyllprednisolone injection through mastoid cribriform area subperiosteum on patients with refractory sudden hearing-loss after failure of routine treatment. METHODS 72 ears in patients suffering from refractory sudden hearing-loss were divided into treatment group and control group. There were 36 ears in treatment group,and 36 ears in control group. Patients in treatment group was injected 40 mg of methypredinisolone sodium succinate through mastoid cribriform area subperiosteum once a time every two days （totally seven times）, the control group was treated with tanakan and mecobalamin by oral administration for 2 weeks. Brainstem electric response audiometry of all patients were examined before and one week after treatment.
