胎儿左侧异构综合征的产前超声诊断价值(附12例报告)

Diagnostic value of prenatal ultrasound of left atrial isomerism

目的探讨胎儿左侧异构综合征的产前超声诊断价值,以提高对左侧异构综合征的认识。方法回顾性分析行产前超声诊断并经病理解剖或新生儿超声检查证实的胎儿左侧异构综合征病例12例,对左侧异构综合征的超声图像特征进行总结。结果 12例产前超声均诊断左侧异构综合征,其主要超声表现为:8例为左位心,4例为右位心;12例均有内脏异构、下腔静脉离断伴奇静脉异位引流;合并房-室传导阻滞5例、心脏结构畸形6例、心脏以外的结构畸形3例。结论左侧异构综合征是一种复杂的综合征,有其特征性的超声表现,产前超声诊断具有极高的临床价值。

Objective To study the diagnostic value of prenatal ultrasound of left atrial isomerism, in order to improve the knowledge of left atrial isomerism. Methods We made a retrospective analysis of 12 fetuses with left atrial isomerism that were diagnosed by prenatal ultrasonic scans and confirmed by autopsy or neonatus ultrasonography, and summarized the ultrasonic features of left atrial isomerism. Results Twelve cases were diagnosed as left atrial isomerism by prenatal ultrasonic. The major ultrasonic findings of left atrial isomerism were as follow： eight of them had levocardia, four had dextrocardia, all had viscerocardiac heterotaxy and interruption of inferior vena cave, five had heart block, six had cardiac anomalies, and three had anomalies outside of the heart. Conclusion Left atrial isomerism is a kind of complex syndrome with characteristic ultrasonic features, and prenatal ultrasonic scan is of great value.
