血清生长趋化因子对脓毒症患者临床预后的价值 被引量:6

摘要 目的探讨细胞生长趋化因子(GRO-α)在脓毒症患者预后评估中的价值和作用。方法收集2011年1月-2012年9月本院ICU中64例脓毒症患者血清,按预后情况分为生存组31例和死亡组33例,建立正常健康对照组50例,分别测定各组血清中GRO-α水平与各组血清降钙素原(procalcitonin,PCT)水平,并进行各组血清GRO-α水平比较、生存组和死亡组血清PCT比较以及血清GRO-α与PCT水平的相关性分析,同时对64例脓毒症患者血清GRO-α水平进行生存分析。结果生存组GRO-α表达水平高于正常对照组,但差异无统计学意义(P>0.05);死亡组的GRO-α表达水平明显高于生存组和正常对照组(P<0.05),死亡组的血清PCT值与生存组比较差异无统计学意义(P>0.05),各组GRO-α和PCT水平无显著的相关性(P>0.05)。生存分析显示高GRO组脓毒症患者生存期明显低于低GRO组(P<0.05)。结论脓毒症患者血清GRO-α水平与患者预后密切相关。与PCT水平相比较,GRO-α水平对脓毒症具有更高的预后评估价值。 Objective To study the value and role of GRO- α l in assessing prognosis of sepsis patients. Methods Sixty-four sepsis patients admitted to ICU in our hospital from January 2011 to September 2012 were divided into survival group (n=31) and death group (n=33), and 50 healthy subjects served as a control group. Their serum GRO- α and procalcitonin (PCT) levels were measured. The survival time of the 64 sepsis patients was analyzed. Results No significant difference was found in the GRO- α expression level although it was higher in survival group than in control group (P 〉 0.05). The GRO-α expression level was significantly higher in death group than in survival group and control group (P 〈 0.05). No significant difference was found in the serum PCT level between death group and survival group (P 〉 0.05). The serum GRO- α level was not significantly related with the serum PCT level (P 〉 0.05). Survival analysis showed that the survival time was shorter in sepsis patients with a high serum GRO- α level than in those with a low serum GRO-α level (P 〈 0.05). Conclusion The serum GRO- α level in sepsis patients is closely related with their prognosis. The value of serum GRO- α level is higher than that of serum PCT level in predicting the survival rate and in assessing the prognosis of sepsis patients.
