摘要
目的探讨多西他赛联合顺铂通过静脉滴注化疗和动脉灌注化疗治疗宫颈癌的临床疗效。方法将我院2011年7月至2014年1月收治的200例宫颈癌患者随机分为观察组和对照组,每组100例。两组患者均予以多西他赛联合顺铂化疗,其中观察组患者在DSA影像指导下进行动脉灌注化疗,对照组患者采取静脉化疗,比较两组患者化疗后的临床疗效以及不良反应发生情况。结果观察组经化疗总有效率为91.00%,对照组为87.00%,观察组略高于对照组,但两组比较无统计学差异(P>0.05);化疗后观察组患者消化道反应发生率为16.00%,骨髓抑制发生率为20.00%,均分别明显低于对照组的73.00%和55.00%,两组比较差异有显著统计学意义(P<0.01);两组患者的肾功能损坏发生情况相比无统计学差异(P>0.05)。结论对宫颈癌患者采取多西他赛联合顺铂化疗具有明显的缓解肿瘤作用,静脉滴注尽管简便易行,但药物毒副反应大,而动脉治疗虽较为复杂,但不良反应少,安全性高,值得临床推广应用。
Objective To investigate the clinical effects of arterial infusion chemotherapy and intravenous chemotherapy with docetaxel and cisplatin for the treatment of cervical cancer. Methods 200 patients with cervical cancer in our hospital from July 2011 to January 2014 were selected and randomly divided into the observation group and the control group, with 100 cases in each group. Both groups were treated with docetaxel combined with cisplatin for chemotherapy, in which, the observation group received arterial infusion chemotherapy under the direction of DSA imaging, while the control group received intravenous chemotherapy. The clinical effect after chemotherapy and the incidence of adverse reactions were compared between two groups. Results The total effective rate was 91.00% and 87.00% of the observation group and the control group respectively, with no statistical difference (P〉0.05). The incidence of gastrointestinal reactions and myelosuppression of the observation group were 16.00% and 20.00% respectively, significantly lower than 73.00% and 55.00% of the control group respectively (both P〈0.01), showing no statistical difference in the incidence of renal damage between the two groups (P〉0.05). Conclusions Chemotherapy with docetaxel and cisplatin has significant ease-tumor effect on treatment of cervical cancer. Intravenous infusion is simple, but the drug toxicity is large; arterial therapy is more complex, but has fewer adverse reactions and higher safety, which is worthy of clinical application.
