改良的ABCD^2评分在短暂性脑缺血发作进展为脑梗死的早期危险评估中的应用

Application of Improved ABCD2Score in the Early Risk Assessment of Transient Ischemic Attack Progressing to Cerebral Infarction

目的探讨改良的ABCD2评分对短暂性脑缺血发作进展为脑梗死的早期危险评估中的价值。方法 160例TIA住院患者,分别用ABCD2评分量表和改良的ABCD2评分量表进行评分和危险分组,记录各个危险组发病7天内的脑梗死例数,对比两个量表中高危组的脑梗死率。结果经过1周的住院治疗后,160例病人中进展为脑梗死的例数一共为17例。ABCD2评分量表筛选出高危组例数为72例,进展为脑梗死10例,脑梗死率13.89%,而改良的ABCD2评分量表筛选出高危组例数为49例,进展为脑梗死14例,脑梗死率28.57%,从两者的高危组脑梗死率比较,改良ABCD2评分量表显著高于ABCD2评分量表,P<0.05。结论改良的ABCD2评分量表在TIA进展为脑梗死的早期危险评估中,与ABCD2评分相比,提高了准确性,可以推广应用。

Objective To investigate the value of improved ABCD^2 score in the early risk assessment of transient ischemic attack （TIA） progressing to cerebral infarction. Methods ABCD^2 rating scale and improved ABCD^2 rating scale were used to grade and define risk groups in 160 cases of hospitalized patients with TIA. The number of cases with cerebral infarction from each risk groups was record in the pathogenesis of 7 days, and then cerebral infarction rate in high-risk groups based on two scales respectively were compared. Results After one week of hospitalization, 17 patients progressed to cerebral infarction among 160 cases. ABCD^2 rating scale screened out 72 cases classed into high-risk group, of which 10 cases progressed to cerebral infarction, and the cerebral infarction rate was 13.89%. But improved ABCD^2 rating scale screened out 49 cases classed into high-risk group, of which 14 cases progressed to cerebral infarction, and the cerebral infarction rate was 28.57%. Therefore, the cerebral infarction rate of high-risk group based on improved ABCD2 rating score was significantly higher than that based on ABCD^2 rating scale （P 〈0.05）. Conclusions In the early risk assessment of TIA progressing to cerebral infarction, improved ABCD^2 rating scale has improved accuracy compared with ABCD^2 score and is worthy of promotion and application for the future.
