不同方法检测HBeAg低值血清的效果评价

Different methods detecting low quantitative HBeAg in serum

目的 通过检测HBeAg低值血清,评价ELISA方法和时间分辨免疫荧光法（IrMA）的检测效果.方法 对2014年1月至6月我院HBeAg阳性结果进行回顾性分析,同时收集门诊和住院受检人群化学发光法（CMIA）检测HBeAg结果为1.00～29.99 S/CO区间内的低值血清样本81份,采用ELISA和IFMA法进行复孔检测.结果 HBsAg阳性人群的HBeAg检测结果与HBsAg呈正相关（r=0.680,P＜0.01）.HBeAg阳性样本占HBsAg阳性样本的32.69％,HBeAg低值样本占HBeAg阳性样本的39.35％,均主要分布于青中年人群.HBeAg阳性率随HBsAg阳性人群的年龄增高而降低（x2=386.041,P＜0.01）.ELISA和IFMA法检测HBeAg低值样本的阳性率分别为60.49％和45.68％,两者比较差异无统计学意义（x2=3.569,P＞0.05）.两种方法在检测1.00～4.99S/CO、5.00～29.99 S/CO水平的阳性率分别为25.64％、92.86％（ELISA）和10.26％、78.57％（IFMA）.相关性分析中,ELISA和IFMA法检测HBeAg低值样本的结果与CMIA方法检测结果均呈明显正相关（r=0.868和0.858,P＜0.01）.结论 由于方法灵敏度方面的原因,ELISA和IFMA法在检测HBeAg低值样本时存在较高比例的漏检情况,因此上述两种方法不适用于检测HBeAg低值样本以评估受检者感染HBV的疾病状况.

Objective To evaluate the performance of ELISA and time-resolved immunofluorometric assay （IFMA） detecting low quantitative HBeAg in serum.Methods The positive results of HBeAg were retrospectively analyzed from Janurary to June 2014 at our hospital.81 serum samples with the HBeAg concentration ranging from 1.00 to 29.99 S/CO were collected and then detected by ELISA and IFMA.Results The quantitation of HBeAg significantly correlated with the quantitation of HBsAg （r=-0.680,P 〈 0.01）.32.69% patients positive in HBsAg were positive in HBeAg.39.35% patients positive in HBeAg had low quantitative HBeAg and all were in young or middle age.The positive rate of HBeAg decreased as the age of patients positive in HBsAg increased （x 2=386.041,P 〈 0.01）.The detection rate of low quantitative HBeAg was 60.49%.by ELISA and 45.68% by IFMA,without statistical difference （x 2=3.569,P 〉 0.05）.At levels of 1.00-4.99 S/CO and 5.00-29.99 S/CO,the detection rate were 25.64% and 92.86% by ELISA and 10.26% and 78.57% by IFMA.All the results of low quantitative HBeAg detected by ELISA and IFMA positively correlated with the results detected by CMIA （r=0.868 and 0.858,P 〈 0.01）.Conclusions Because of the methods＇ sensitive digree,ELISA and IFMA would fail in detecting low quantitation of HBeAg high proportionally.Therefore,it＇ s not suitable for evaluating the situation of the patients infected by HBV detecting low quantitation of HBeAg by these two methods.
