强化干预对高血压人群高敏C反应蛋白的影响

Effect of intensive intervention on high-sensitivity C-reactive protein in patients with hypertension

摘要 目的:分析强化干预对高血压人群高敏C反应蛋白(hs-CRP)水平的影响。方法:选取开滦集团公司井下及井下辅助单位的高血压职工为强化干预组(4 474例),给予综合降压干预,包括宣传教育、发放降压药物、工会参与和随访。同时选取开滦集团公司非井下单位高血压职工为非强化干预组(4 055例),只给予宣传教育。分别于2008-2009和2010-2011年度健康体检时测定hs-CRP水平,观察两组干预前后hs-CRP水平的变化情况,并对hs-CRP水平下降的影响因素进行logistic回归分析。结果:强化干预组和非强化干预组hs-CRP水平均较干预前下降。经协方差分析,调整后的hs-CRP下降值分别为(0.24±0.03)和(-0.04±0.03)mg/L,差异有统计学意义(P<0.01)。经logistic回归分析显示,强化干预组hs-CRP水平下降的概率为非强化干预组的1.33倍。结论:强化干预可降低高血压人群的hs-CRP水平。 Objective:To analyze the effect of intensive intervention on high-sensitivity C-reactive protein (hs- CRP) in patients with hypertension. Method: Employees with hypertension on-the-job from underground and un- derground auxiliary units of Kailuan Group were enrolled for the intensive intervention group (n=4 474) and re- ceived intensive intervention for hypertension,including propaganda and health education, taking antihypertensive medications, labor union participation and follow-up visit. At the same time employees with hypertension on-the- job from overground units of Kailuan Group were enrolled for non-intensive intervention group (n= 4 405) and re- ceived propaganda and health education. Hs-CRP was measured and observed before and after intervention,and the factors contributing to the decline of hs-CRP post-intervention was analyzed by logistic regression analysis. Result: Subjects in every group exhibited significant reductions in hs-CRP compared with baseline. Hs-CRP decreased by (0.24±0.03) and (0.04±0.03)mg/L in intensive intervention group and non-intensive intervention group re- spectively. The decline of hs-CRP had significant statistical difference between two groups. Logistic regression a- nalysis demonstrated that the probability of decreased hs-CRP in the intensive intervention group was 1.33 times higher than that in non-intensive intervention group. Conclusion: Hs-CRP can be reduced by intensive intervention for hypertension.
