卵巢囊腺瘤与囊腺癌的CT征像及病理对照

CT Signs Compare with Pathological Diagnosis on Ovarian Cystadenoma and Cystadenoearcinoma

目的观察卵巢囊腺瘤与囊腺癌的CT影像特点,并与病理对照,旨在提高CT诊断的准确率。方法回顾性分析20例经手术及病理证实的卵巢囊性及囊实性占位患者的病例资料,总结其CT表现和特征,并和病理分型进行对照。结果20例中囊腺瘤7例,其中浆液性囊腺瘤1例,黏液性囊腺瘤5例,混合性囊腺瘤1例,囊腺瘤表现为薄壁的单房或多房囊性肿物,形态规则,囊液呈均匀的液体密度,无壁结节。交界性囊腺瘤4例,以囊性为主,伴有分隔。囊腺癌7例,其中2例为双侧,共9个病灶,其中浆液性囊腺癌8个,粘液性囊腺癌1个,以囊实性为主,多不规则形,囊壁及囊内分隔厚而不规则呈结节状。子宫内膜样腺癌2例(其中有1例并发对侧粘液性囊腺瘤),分型不明的腺癌1例,均以囊实性为主。结论CT能显示卵巢囊腺瘤及囊腺癌的特征,并能与一些常见的较典型的卵巢囊实性病变相鉴别,可为临床提供手术和治疗方案的依据。

Objective To observe CT signs of ovarian cystadenoma and cystadenocarcinoma with pathological diagnosis, in order to improve the accuracy of CT diagnosis.Methods To analyze the clinical data of 20 cases with ovarian cystadenoma and cystadenocarcinoma proved by surgery and pathology retrospectively.Results Of the 20 cases, cystadenocarcinomas were 7 cases; among them, 1 was serous cystadenoma, 5 were mucinous cystadenoma, 1 was mixed cystadenoma. The CT findings of cystadenoma were cystic with thin wall, regular in shape, unilocular or multilocular, equal liquid density, lack of the mural nodule. 4 cases were borderline cystadenomas, mainly cystic,accompanied with septum in the cyst. 7 cases were cystadenocarcinoma, of which, 2 cases with double side, a total of 9 lesions, included 8 serous and 1 mucous. The cystadenocarcinomas were always cystic and solid, bilateral, irregular in shape with thick and irregular wall. 2 cases were endometrioid adenocarcinoma, among them, one accompanied with mucinous cystadenoma in the opposite ovary; type unidentified adenocarcinoma was 1, all of these three adenocarcinomas were mainly cystic and solid.Conclusions CT scanning is a valuable means in diagnosing of ovarian cystadenoma and ovarian cystadenocarcinoma, it can also provide differential diagnosis between some of the common and typical ovarian cystic and solid lesions. In addition,it will provide basis for surgery and treatment in clinical.
