分离性垂直斜视术后远期效果分析

Analysis the effectiveness of the treatment of DVD with anterior transposition of the inferior oblique muscle

目的 探讨分离性垂直斜视（DVD）患儿术后远期效果.方法 回顾性分析2008年2月至2012年4月在南昌大学第二附属医院儿童眼科住院手术的分离性垂直斜视患儿36例（66只眼）,其中30例双眼DVD,6例单眼DVD.24例伴有上斜视,8例伴有内斜视,4例伴有外斜视,均合并不同程度的下斜肌功能亢进.33例行单纯下斜肌转位术,3例行下斜肌转位术联合上直肌后徙术.12例合并水平斜视者,其中10例行一期手术,2例行二期手术.观察术后1年的眼位、下斜肌功能、代偿头位、睑裂、眼球运动、同视功能及并发症情况.结果 术后1年29例（80.56％,29/36）患儿术后下斜肌亢进消失,恢复满意,7例（19.44％,7/36）改善;25例（69.44％,25/36）恢复双眼单视功能,其中19例（76％,19/25）恢复同时视功能,4例（16％,4/25）恢复融合功能,2例（8％,2/25）恢复立体视.结论 手术是治疗分离性垂直斜视的常用方法,下斜肌转位术后眼球上转受限和睑裂变化不明显,且能纠正10～35△的垂直斜视,是治疗伴有下斜肌功能亢进分离性垂直斜视的较佳方法.

Objective To analyze the effectiveness of the DVD treatment with anterior transposition of the inferior oblique muscle （ATIO）.Methods The surgery of ATIO was performed on 36 patients （66 eyes） from 2008 to 2012,but with superior rectus recession combined with anterior transposition of the inferior oblique muscle in 3 cases.Results The inferior oblique overaction disappeared after the operation about 29 cases （80.56%,29/36）,improved after the operation about 7 cases （19.44%,7/36）.Conclusions ATIO is an effective method for the treatment of DVD with inferior oblique overaction,especially in deviation of more than 10△.
