摘要
目的探讨神经内镜辅助下微血管减压术(MVD)治疗原发性三叉神经痛(ITN)的方法及临床效果。方法回顾性分析2011年1月至2012年6月采用神经内镜辅助下MVD治疗24例ITN患者的临床资料。结果 24例ITN患者中,术后22例(91.7%)疼痛完全消失;1例(4.2%)疼痛明显缓解;1例(4.2%)并发面部麻木、感觉减退,6个月后完全消失。本组无术后感染、脑出血、脑梗死、死亡。结论神经内镜辅助下MVD治疗ITN,可以消除桥小脑角区显微手术的解剖死角,避免遗漏责任血管、协助调整Teflon垫片位置,能提高手术有效性,减少并发症的发生。
Objective To explore the method to treat primary trigeminal neuralgia (PTN) by neuroendoscope-assisted microvascular decompression and its curative effect on PTN. Methods The clinical data of 24 patients with PTN undergoing neuroendoscope-assisted microvascular decompression from January, 2011 to June, 2012 were analyzed retrospectively, including the operative method and the following-up data. Results After the surgery PTN disappeared completely in 22 patients and FFN was significantly relieved in 2 and the postoperative facial numbness and hypesthesia occurred after the surgery and disappeared 6 months after the surgery in a patient with postoperative relief of PTN. Conclusions The anatomical structure in the cerebellopontine angle may be clearly exposed by the neuroendoscope-assisted microvascular decompression in which the vessels responsible for PTN can not be omitted and the Teflon pad position was easily adjusted. And the postoperative complications may be decreased and the curative effect of the decompression on PTN may be enhanced by the neuroscopes assistance.
