症状性颈动脉完全闭塞患者血管再通术的临床疗效及相关影响因素分析 被引量:2

摘要 目的探讨对症状性颈动脉完全闭塞患者实施血管再通术的安全性及有效性,且对相关影响因素进行分析。方法选取2009年1月—2012年2月期间神经内科收治的缺血性脑卒中患者120例,对其中40例进行颈动脉闭塞血管再通术,失败8例,予以剔除,剩余32例列入观察组,药物治疗组80例列为对照组,对两组的日常行为能力Barthel指数评分进行比较,采用t检验或秩和检验进行统计,P<0.05为差异有统计学意义。结果共进行颈内动脉闭塞血管再通术40例,成功32例,手术成功率为80.00%;2组随访结果为,32观察组患者中,死亡1例,失访2例,80例对照组病例中,死亡6例,其中4例死于脑梗死,2例死于肺炎,失访4例;观察组患者Barthel指数在随访3个月、6个月、1年及2年时的功能预后评分的平均秩次均低于对照组,差异有统计学意义(P<0.05);以Barthel指数评分结果为应变量,以患者的基本情况,包括吸烟、介入治疗、美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分、糖尿病、高血压等为自变量,进行逐步logistic回归分析,结果显示吸烟、NIHSS评分与治疗方式(介入或药物)入选方程,表明Barthel指数评分与患者是否吸烟、NIHSS评分情况及治疗方式具有显著相关性,差异有统计学意义(P<0.05)。结论对症状性颈动脉完全闭塞患者实施血管再通术的介入性治疗可显著提高临床疗效,同时相关影响因素分析结果显示,吸烟与NIHSS评分不利于神经功能的恢复,而介入治疗则有利于患者预后。
