帽状腱膜下针刺法治疗慢性紧张性头痛26例临床观察

A Clinical Observation on Subgaleal Acupuncture for 26 Cases of Chronic Tension Headaches

目的观察帽状腱膜下针刺法治疗慢性紧张性头痛的临床疗效。方法将50例慢性紧张性头痛患者随机分为治疗组（26例）和对照组（24例）。治疗组采用帽状腱膜下针刺法,主穴：百会、神庭、头维（双侧）、太阳（双侧）、风池（双侧）、太冲（单侧）,头部穴位先斜刺到帽状腱膜下再平刺,进针0.5-0.8寸,并捻转,头痛不发作时采用中度刺激,头痛发作时采用重度手法捻转;其他穴位直刺,采用平补平泻法,留针30min。对照组取穴同治疗组,头部穴位平刺0.3~0.5寸,其他穴位直刺,均采用平补平泻手法,留针30min。两组均每周治疗3次,共治疗12周,24周时随访。于治疗前后及随访时进行头痛程度评分、头痛持续时间评分,计算头痛指数,并观察临床疗效。结果两组治疗后和随访时头痛程度评分、疼痛持续时间评分、头痛指数均较本组治疗前明显降低（P〈0.05）;治疗组治疗后和随访时头痛程度评分、疼痛持续时间评分、头痛指数均低于同时间点对照组（P〈0.05）。治疗组临床疗效总有效率为92.3%,对照组为83.3%,两组比较差异无统计学意义（P〉0.05）。结论帽状腱膜下针刺法治疗慢性紧张性头痛能明显改善头痛程度及持续时间,较常规进针法为优。

Objective To observe the efficacy of subgaleal acupuncture for chronic tension headache.Methods Fifty cases of chronic tension headache were randomized into the treatment group of 26 cases and control group of 24 cases.The treatment group was given subgaleal acupuncture.The control group was given common acupuncture.The acupoints such as Baihui（ GV20）,Shenting（ GV24）,Touwei（ ST8,bilateral）,Taiyang（ EX-HN5,bilateral）,Fengchi（ GB20,bilateral） and Taichong（ LR3,unilateral） were selected as the main points in both groups.Both groups were treated 3 times weekly for 12 weeks.In the 24 th week,the patients were followed up.The headache severity score and headache duration score were observed.The headache index was calculated and the clinical efficacy was observed.Results The headache severity score,headache duration score and headache index were significantly decreased in both groups after treatment（ P〈0.05）,especially in the treatment group（ P〈0.05）.The total effective rate of the treatment group and control group was 92.3% and 83.3% respectively,with no significant difference between groups（ P〉0.05）.Conclusion Subgaleal acupuncture for chronic tension headache is superior to common acupuncture in relieving headache severity and headache duration.
