鼻咽癌患者术后颅内感染的危险因素分析

Analysis of the related risk factors for postoperative intracranial infection in patients with nasopharyngeal carcinoma

目的探讨鼻咽癌患者术后发生颅内感染的相关危险因素,为临床防治提供理论依据。方法选择医院2008年12月-2012年12月收治的98例鼻咽癌患者临床病例进行回顾性研究,观察患者发生颅内感染情况,并对发生颅内感染患者与未发生颅内感染患者的临床资料进行对比分析,分析鼻咽癌患者发生颅内感染的相关危险因素。结果 98例鼻咽癌患者发生颅内感染14例,感染率14.29%;经单因素分析发现,患者的年龄、病程、手术入路方式、手术时间、合并疾病、鼻咽癌的病理类型以及肿瘤部位与颅内感染的发生有显著的相关性(P<0.05);进一步进行多因素logistic回归分析发现,患者的年龄、病程、手术方式、病理类型以及肿瘤部位是发生颅内感染的独立危险因素。结论导致鼻咽癌患者术后发生颅内感染的因素较多,临床医师需要根据患者的自身情况选择恰当的手术方式,并对相关因素进行针对性干预,以减少感染的发生。

OBJECTIVE To investigate risk factors associated with postoperative intracranial infection in patients with nasopharyngeal carcinoma , so as to provide a theoretical basis .METHODS Totally 98 patients with nasopharyngeal carcinoma from Dec .2008 to Dec .2012 in our hospital were studied .The situation of intracranial infection was observed ,and the clinical data of patients with intracranial infection and without intracranial infection were compared to analyze risk factors associated with intracranial infection in patients with nasopharyngeal carcinoma .RESULTS Totally 14 out of 98 patients with nasopharyngeal carcinoma had intracranial infection ,the infection rate was 14 .29% .The univariate analysis showed that the patient′s age ,disease duration ,surgical approach ,surgical time ,concomitant diseases ,pathological types of nasopharyngeal carcinoma and tumor sites were significantly correlated with intracranial infections （P〈0 .05） .The multivariate logistic regression analysis revealed that the patient′s age ,disease duration ,surgical approach ,pathological types and location of the tumors were the independent risk factors for intracranial infections .CONCLUSION There are many factors leading to postoperative intracranial infections in patients with nasopharyngeal carcinoma .Clinical physicians should choose the proper surgical approach according to the status of the patients and perform interventions targeted at related factors to reduce infections .
