肾集合管癌CT表现

CT manifestations of renal collecting duct carcinoma

目的探讨肾集合管癌（CDC）的CT特征。方法回顾性分析11例经我院手术病理证实的CDC患者CT资料,观察CDC的CT特征。结果所有患者均为单侧发病,肿瘤边界不清,形态不规则,直径3-12cm;8例（8/11,72.73%）呈单纯浸润性生长,3例（3/11,27.27%）呈局部膨胀性生长伴浸润性生长;4例（4/11,36.36%）累及肾皮质和髓质,7例（7/11,63.64%）累及肾皮质、髓质及肾盂;7例（7/11,63.64%）呈囊实性,4例（4/11,36.36%）呈实性;增强扫描病灶实性部分呈不均匀轻-中度强化,强化程度低于肾皮质;5例（5/11,45.45%）肾脏体积增大或局部轮廓发生改变;1例（1/11,9.91%）肿瘤内见斑点状钙化;3例（3/11,27.27%）侵犯肾周筋膜,5例（5/11,45.45%）肾门淋巴结转移。结论 CDC的CT表现具有一定特征,有助于临床诊断。

Objective To explore CT features of renal collecting duct carcinoma （CDC）. Methods CT data of 11 patients of CDC confirmed by pathology were analyzed. Results All the tumors were unilateral with unclear boundaries and irregular shapes, and the diameter ranged from 3 to 12 cm. There were 8 tumors （8/11,72.73%） showing invasive growth and 3 （3/11,27.27%） showing local swelling growth combined with invasive growth. The renal cortex and medulla were involved in 4 cases （4/11, 36.36%）, and the renal cortex, medulla and renal pelvis were involved in 7 cases （7/11, 63.64%）. The tumor was cystic-solid in 7 cases （7/11, 63.64%） and solid in 4 cases （4/11, 36.36%）. In enhanced scanning, the solid components of the tumors presented heterogeneous mi!d-moderate enhancement, which was weaker than renal cortex. Enlargement of the renal or change of the local renal contour was observed in 5 case （5/11, 45.45%）. The punctate calcification inside the tumor was found in 1 case （1/11, 9.91%）. The perirenal fascia was involved in 3 cases （3/ 11, 27.27%） and renal hilum lymph nodes metastasis occurred in 5 cases （5/11, 45.45%）. Conclusion CT manifestations of CDC has certain characteristics, which is helpful for clinical diagnosis.
