复合蛋白营养剂治疗乙型肝炎肝硬化患者营养状态及肝功能的变化 被引量:4

摘要 目的观察复合蛋白营养剂对存在蛋白质-能量营养不良(PEM)的乙型肝炎肝硬化患者营养状态及肝功能的影响。方法选择存在PEM的乙型肝炎肝硬化患者40例,基线均不存在相关并发症,分为复合蛋白营养剂组25例及对照组15例。复合蛋白营养剂组给予新型夜间加餐(原味酸牛奶200g+复合蛋白营养剂15g)配合一般综合治疗,对照组给予一般综合治疗;观察两组患者的血清白蛋白、前白蛋白、淋巴细胞计数,并记录患者发生并发症的情况。结果在两组患者基线资料具有可比性的情况下,与治疗前相比,复合蛋白营养剂组患者血清白蛋白、前白蛋白、淋巴细胞计数明显上升,在6周末,对照组患者白蛋白为(30.2±3.2)g/L,显著低于治疗组[(33.6±3.7)g/L,P<0.05],前白蛋白为(122.10±24)g/L,显著低于治疗组[(159.0.6±30.4)g/L,P<0.05],淋巴细胞数为(1.08.10±0.31)×109/L,显著低于治疗组[(1.36±0.26)×109/L,P<0.05],并发症发生较少,与对照组相比其差异均有统计学意义(P<0.05)。结论复合蛋白营养剂可以改善存在PEM的乙型肝炎肝硬化患者的营养状态及肝功能,有可能减少患者并发症的发生。 Objective To investigate the effect of compound protein nutrition agent on nutritional status and liver function of hepatitis B virus(HBV)-related cirrhotic patients with protein-energy malnutrition(PEM). Methods Forty HBV-related cirrhotic patients with PEM were enrolled and no other complications existed at the baseline.Patients were then divided into protein nutrition agent group(intervention group,25 cases) and control group(15cases). Patients in the intervention group were given evening snack(200g yoghourt and 15 g compound protein nutrition agent) with general comprehensive treatment,while patients in the control group only received general treatment. Serum albumin,prealbumin and lymphocyte count were tested and complications were recorded as well.Results Baseline characteristics of the two groups were comparable,and the serum albumin,prealbumin and lymphocyte count in patients of intervention group were significantly increased than that of the controls. At 6weeks,the albumin,prealbumin and lymphocytes counts in patients of the control group were(30.2 ±3.2) g/L,(122.10 ±24) g/L and(1.08.10 ±0.31)×10^9/L,significantly lower than the intervention group [(33.6 ±3.7)g/L,(159.0.6±30.4)g/L and(1.36±0.26)×10^9/L,P〈0.05]. Complications in intervention group were less than the control group(P〈0.05).Conclusions Compound protein nutrition agent nutrients can improve the nutrition status and liver function of HBV-related cirrhotic patients with protein-energy malnutrition and may reduce their complications.
