Effects of mechanical ventilation with different tidal volumes on TNF-α and IL-10 in lung tissue of rats

摘要 目的观察不同潮气量机械通气对大鼠肺组织中TNF-α和IL-10的影响及其作用机制。方法选取24只SD大鼠并随机分成三组(F组、L组、H组),每组8只,其中F组为插管不通气组,L组给予10 ml/kg小潮气量,H组给予40 ml/kg大潮气量行机械通气。L组与H组的通气时间均为2 h,并在通气的过程中给予维库溴铵抑制呼吸。通气结束24 h后,取各组大鼠的肺组织检测湿干重比值(W/D值),用ELISA法观察肺组织匀浆中细胞因子(TNF-α,IL-10)蛋白的浓度,观察肺组织病理改变并分析具体的作用机制。结果 L组和H组机械通气后肺组织的TNF-α、IL-10蛋白含量表达均较F组有增高,其中H组与F组比较差异有统计学意义(P<0.05),L组浓度虽有升高,但差异无统计学意义(P>0.05),此外H组较L组的升高变化也非常明显,差异有统计学意义(P<0.05)。结论大量的机械通气会对大鼠造成明显的肺损伤,从而引发炎症反应,使得炎性因子TNF-α和IL-10含量产生变化,由此可以得出不同机械通气量与大鼠的促炎及抑炎反应有明显相关性的结论。 Objective To observe the effects of different tidal ventilation volumes on TNF-αand IL-10 in lung tissue. Methods A total of 24 SD rats were randomly and equally divided into 3 groups (group F, group L, and group H). Group F was non-ventilation group, while group L and group H were given 10 ml/kg tidal volume and 40 ml/kg large tidal volume mechanical ventilation respectively. Ventilation time of group L and group H was 2 hours, and vecuronium bromide was applied to inhibit respiration during the ventilation process. 24 h after ventilation, the lung tissue was taken for wet to dry weight ratio test (W/D value), and the concentrations of cytokines (TNF-αand IL-10) protein in lung tissue homogenates were detected by ELISA. Meanwhile, the pathological changes of lung tissue were observed and the underlying specific mechanism was analyzed. Results After mechanical ventilation, the expres-sions of TNF-αand IL-10 proteins in lung tissue in group L and group H were higher than those in group F, with signifi-cant difference between group H and group F (P〈0.05), but not between group L and group F (P〉0.05). Moreover, those proteins in group H showed statistically significant higher levels than those in group L (P〈0.01). Conclusion Mechani-cal ventilation amount will cause significant lung damage in rats, which leads to inflammation as well as changes of in-flammatory factor TNF-αand IL-10. Therefore, different mechanical ventilation volumes are regarded to be closely related to proinflammatory and anti inflammatory reaction in rats.
