尿微量清蛋白检测在糖尿病肾病早期诊断中的临床应用

Clinical application of detection of urinary microalbumin in the early diagnosis of diabetic nephropathy

目的探讨尿微量清蛋白在糖尿病肾病早期诊断中的应用价值,为临床早期诊断提供一定的理论依据。方法选取本院内分泌科收治的110例确诊为2型糖尿病的患者作为观察组,并选取同期于本院体检中心体检的健康人群70例作为对照组。对比两组研究对象尿微量清蛋白定量检测结果和阳性率,同时对比不同病程的患者尿微量清蛋白定量检测结果和阳性率。结果观察组患者尿微量清蛋白定量检测结果为（58.96±8.69）mg/L,阳性率为46.36%,均显著高于对照组（P12年的2型糖尿病患者的尿微量清蛋白定量检测结果为（79.05±7.03）mg/L,阳性率为84.62%,均显著高于病程6-12年和病程〈6年的患者（P〈0.05）;病程6-12年患者的尿微量清蛋白定量检测结果为（34.16±5.42）mg/L,阳性率为46.51%,显著高于病程〈6年的患者（P〈0.05）。结论尿微量清蛋白的检测在糖尿病肾病早期诊断中具有重要的应用价值,同时,动态检测其水平变化对糖尿病肾病的临床治疗和病情评估均有指导意义。

Objective To explore the application value of urinary microalbumin in early diagnosis of diabetic nephropathy, in order to provide certain theoretical basis for the early clinical diagnosis of the disease. Method 110 patients with type 2 diabetes were selected as the observation group, at the same time 70 subjects received health examination were selected as control group in our hospital. The determination of urinary microalbumin and the positive rate in the two groups were compared, and the same indexes were compared among patients with different course. Result In observation group, the quantitative determination of urinary microalbumin was （58.96±8.69） mg/L, the positive rate was 46.36%, they were significantly higher than that of control group （P 〈 0.05）. The quantitative determination of urinary microalbumin in type 2 diabetic patients with course was more than 12 years was （79.05±7.03） mg/L, the positive rate was 84.62%, significantly higher than that of the course between 6 to 12 years and less than 6 years （P 〈 0.05）; the quantitative determination of urinary microalbumin in type 2 diabetic patients with course of 6 to 12 years was （34.16± 5.42） mg/L, the positive rate was 46.51%, significantly higher than that of course less than 6 years （P〈0.05）. Conclusion The detection of urinary microalbumin has important value in the early diagnosis of diabetic nephropathy, dynamic detection of clinical treatment and disease of diabetic nephropathy has guiding significance for evaluation.
